| 설명 | 심신장애자에 대하여 직장-학교-가정 등에서 일반적인 사회생활을 할 수 있게 가능한 한 최대한도까지 기능을 회복시키는 일. 제1차 세계대전 후, 전상자에 대한 회복처치로서 미국과 유럽 여러 나라에서 실시된 것이 근대적 재활의 시초이며, 1940년대 급격한 발전을 하였다. 1948년 세계보건기구(WHO)에 따르면, 재활은 제1상에서 보건-육아, 제2상의 예방의학, 제3상의 치료의학에 이어서 제4상으로 내세우고 있다. 먼저 지체부자유아 문제가 오래 전부터 주목되어 왔고, 교통재해와 산업재해의 증가, 인구의 노령화에 따른 뇌혈관질환의 증가, 사회생활의 복잡화에 따른 정신장애의 증가 등으로 인하여 재활의 수요가 늘어났기 때문에 이에 대한 시급한 대책을 강구하지 않을 수 없게 되었다. 이것은 치료의학에 직결된 의학재활(medical rehabilitation)과 행정적 갱생지도에 의한 직업재활(vocational rehabilitation)로 나누어진다. |

| 설명 | 어떤 범위의 주파수의 음파가 청각기를 자극하여 생기는 감각. 음파의 주파수는 20Hz에서 2만 Hz(1Hz는 매초 1사이클)까지이고, 청각기관은 바깥귀-가운데귀-속귀로 구분된다. 청각 수용기는 속귀의 달팽이관에 있고, 바깥귀 및 가운데귀는 음을 속귀에 전달하는 전음기傳音器이다. 바깥귀는 귓바퀴와 바깥귀길로 이루어져 있으며, 고막이 가운데귀와의 경계를 이룬다. 가운데귀는 망치뼈-모루뼈-등자뼈라고 하는 세 개의 귓속뼈가 있어 고막의 진동을 달팽이관의 안뜰창에 전달한다. 이 때 음파의 진폭은 감쇠되지만 음압은 약 20배로 증강된다. 달팽이관은 조개껍데기의 나사 모양의 골관이며, 횡단면에서는 기저막과 안뜰막에 의해 세 방으로 나누어진다. 양막으로 둘러싸인 부분이 달팽이관인데 내림프로 가득 차 있고, 그 위에 있는 안뜰계와 밑에 있는 고실계는 외림프로 가득 차 있다. 기저막 위에는 털세포가 배열되어 있다. 안뜰창에 전달된 음파는 안뜰계의 외림프를 통해서 달팽이세관의 내림프로 전해지고 끝에 가서는 기저막 위에 털세포에 진동자극을 주게 된다. 털세포가 흥분하면 여기에 부착해 있는 달팽이관신경으로 충동이 발사된다. 달팽이관 신경은 제8뇌신경의 일부로서 숨뇌에 들어가 숨뇌의 올리브핵, 중간뇌의 아래언덕, 안쪽무릎체에 의해서 연접전달을 하여 대뇌겉질 관자엽의 청각부에 이르러 청각을 발생시킨다. |

| 설명 | 일종의 확성장치로서 난청을 보충하기 위해 귀에 장착하는 기구. 옛날에는 나팔형의 집음기였던 것이 지금은 보통 음의 증폭을 돕는 전기보청기가 쓰인다. 꼭지에 장착하는 뼈전도도 있으나 대개는 공기전식으로 된 것이 많다. 즉, 음파를 마이크로폰으로 받아 전기진동으로 바꾸어 이것을 증폭기로 확대하여 이어폰으로 다시 음파로 만들어 귀에 들리게 한다. 난청 가운데서도 전음성 난청에는 효과가 있으나, 신경계 기능저하가 원인인 속귀와 관련된 감음성 난청에는 별로 효과를 기대할 수 없다. 보청기의 사용한계는 잘 들리는 쪽 귀의 평균청력손실 30dB(정상청력의 1/30)을 기준으로 하여 이보다 경도일 때는 필요가 없고 그 이상의 장애가 있을 때 적용된다. 보청기는 트랜지스터에서 IC 회로로 전자기술의 진전과 함께 개량되어 감도도 좋을뿐더러 아주 작아지고, 증폭장치부를 여러 형태로 만든 것이 판매되고 있다. 예를 들면, 안경테에 세트되어 있는 안경형, 귀에 걸게 되어 있는 귀뒤형, 여자의 머리핀 형이나 넥타이핀 형, 포켓형 등이 있다. |

| 설명 | 귀가 들리는 정도를 측정하는 검사. 청각검사라고도 한다. 소리나 목소리가 잘 들리는지의 여부 등 청각의 예민한 정도를 측정하는 일이다. 검사에는 청력계나 소리굽쇠 등의 기구에 의해서 얻는 음원, 즉 순음을 사용하며, 사람의 목소리나 시계소리 등도 음원으로 사용된다. 또 음파가 전파되는 매질에 따라 기도 청력검사와 골도 청력검사로 구별되기도 한다. 검사의 내용으로는 청각의 상하음계나 최소 가청문턱값의 검사를 비롯하여, 충분히 들리는 소리를 사용하여 그것이 들리는 상태를 조사하는 문턱값검사(소리의 크기와 상태의 검사, 세기의 판별역검사 등) 외에 음향자극의 반응으로부터 타각적으로 조사하는 방법도 있으며, 이것은 주로 갓난아기나 정신병환자에 대하여 사용된다. 주로 청력계가 사용되고, 청력은 데시벨(dB)로 표시된다. 정상인은 0dB이고, 난청자일수록 그 수가 커지며, 60dB이상은 상당히 난청이고, 80dB 이상이면 귀머거리이다. |
