| 설명 | 섭취한 음식물 중의 영양소가 가수분해에 의해서 작은창자의 점막면을 통과할 수 있을 정도의 작은 분자로 바뀌는 작용 또는 그 과정이다. 그리고 작은창자의 점막면을 통과해 혈액이나 림프액으로 들어가는 것을 흡수라고 한다. 음식물이 입으로 들어가면 씹어서 잘게 부수고 으깨는 동안에 침이 다량으로 분비되며, 음식물의 주체를 이루는 녹말은 침 속의 ptyalin: (amylase)에 의해 일부가 가수분해되어 maltose와 dextrin이 된다. 침은 음식물이 구강 점막에 닿으면 미각신경이 자극을 받아 반사적으로 다량 분비되므로 씹힌 음식물을 뭉쳐서 삼키기 쉽게 하는 일 외에도 중요한 역할을 한다. 단백질은 침으로 소화되지 않는다. 단백질이 든 음식물이 구강에서 잘게 부수어져서 위에 이르면 위의 날문부 점막에서 gastrin이라는 호르몬이 분비된다. 이 호르몬이 혈액을 통해서 위에 도달하면, 위점막의 벽세포에서 염산이, 주세포에서 pepsinogen이 분비된다. 이 펩시노겐은 염산에 의해서 활성화하여 pepsin이 되어 단백질의 일부를 펩티드까지 가수분해시킨다. 음식물 속의 지방은 대부분이 중성인데 음식물이 잘게 부서진 상태로 위에 도달하며, 위의 운동으로 휘저어져 잘 섞이게 되고 입자가 다소 작아진다. 위액에는 소량의 지방 분해효소인 리파아제가 포함되어 있는데, 이것은 위내로 역류한 이자액 속에 포함되어 있던 것으로, 위 속이 산성일 때에는 활성을 갖지 못한다. 각 영양소의 본격적 소화는 음식물이 위에서 샘창자 빈창자 상부에 이르러서야 이루어진다. 이들 작은창자 부위에서 소화와 흡수가 거의 완료된다고 보아도 좋으며, 이 부위보다 아래쪽에 있는 작은창자는 나머지를 처리함에 지나지 않는다. 샘창자 및 빈창자는 소화가 왕성하게 이루어지는 여러 가지 조건을 갖추고 있으며, 소화하여 생긴 물질을 능률적으로 흡수할 수 있는 넓은 점막면적을 갖고 있다. 이 점막면에 소화산물이 넓게 닿도록 진자운동, 분절운동, 꿈틀운동 등 소화관 운동이 이루어지고, 그에 따라 흡수가 촉진된다. 어떠한 이유로 어쩔 수 없이 이 부위를 절제해버리면, 하부작은창자가 대상적으로 작용하지만 능률이 저하되는 것은 어쩔 수 없다. 한편 작은창자의 말단부는 담즙산 및 비타민 B12를 흡수하는 데에 필요불가결한 곳이다. 큰창자은 소화와는 관계없지만 물과 전해질을 흡수한다. |

| 설명 | 산성 위액의 작용에 의해서 식도, 위 또는 샘창자 점막이 파괴되어 결손부가 생기는 것. 즉 위벽이 허는 것인데 단 그 원인이 산성위액에 의하였을 때 한하여 소화성 궤양이라 한다. 소화궤양은 위산에 의해서 발생한 궤양을 말한다. 소화궤양의 발생메커니즘은 기본으로 위-샘창자의 보호메커니즘과 손상요인의 불균형으로 발생한다. 소화궤양은 대개 긴지름 0.6cm 이상인 경우가 많고, 절반이상은 긴지름이 2cm 이상이다. 전통적으로 소화궤양은 둥근 모양에 경계가 분명하며 궤양의 벽은 반듯하다. 궤양의 가장자리는 악성 궤양과 달리 두꺼워져 있지 않다. |
