| 설명 | 주로 소아에서 일어나고, 그 결과로 지속성 면역을 주는 전염성의 paramyxovirus 병. 흡기에 의해서 감염되는데, 가장 농후한 바이러스감염은 침샘내에 생기며, 특히 턱밑샘이나 혀밑샘보다도 귀밑샘이 더욱 심하다. 잠복기는 18~22일이다. 감염은 약 75%의 증례에서 증상을 나타낸다. 이들 중 귀밑샘염은 70%에서, 수막염은 10~15%(이들 중 반수에서 무증상성 수액세포 증가증이 동반된다)에서 일어난다. 부고환고환염이 사춘기 후의 남성에서 생기지만 그 후에 불임증을 속발하는 일은 드물다. 그 외의 증상은 보다 드물게 나타나는데 이자염, 관절염, 심근염, 난소염, 갑상샘염 및 젖샘염 등이 있다. 발열과 환부의 유통성 염증은 최초의 2일간에 가장 현저하고 다음 4~5일이 지나면 서서히 가라앉는다. 동시에 한 부위 이상을 침습하는 수도 있으며 가끔 속발적으로 침습되고 질환의 전 경과는 2~3주에 이른다. 부수되는 영속성의 신경학적 손상을 동반하는 수막뇌염은 드물다. |

| 설명 | 산부인과 영역에서 가장 빈번한 응급 병으로, 그 발생 빈도는 매년 증가하는 추세이다. 자궁외 임신은 수정란이 정상적인 위치인 자궁몸통의 내강에 착상되지 않고 다른 곳, 즉 난소에서 나온 난자를 자궁까지 운반하는 자궁관, 난자를 생산하는 난소, 자궁을 지지하는 여러 인대, 복강, 자궁의 입구에 해당하는 자궁경부 등에 착상되는 임신을 말한다. 착상하는 부위에 따라 자궁관 임신, 자궁목관 임신, 난소 임신, 복막임신, 인대내 임신 등으로 나뉘며, 가장 흔한 자궁외 임신은 자궁관 임신이다. 자궁외 임신이 증가하는 이유로는 우선 불결한 성적 접촉에 의해 전파되는 급성 난관염의 증가를 생각할 수 있다. 실제 급성 난관염을 앓았던 여성은 그 다음 임신에서 자궁외 임신이 될 확률이 정상인에 비하여 7배나 높은 것으로 알려져 있다. 또한 자궁내 피임장치를 사용하는 여성에서도 정상인보다 높은 율의 자궁외 임신이 보고되었다. 자궁외 임신이 되면 반드시 다양한 형태의 하복부 동통을 나타낸다. 대부분 최종 월경일을 기준으로 4주쯤 후부터 부정기 질출혈을 보인다. 또 과반수의 환자에서 유방통이나 멀미 등이 나타나기도 하나, 체온 상승은 드물다. 그러나 빠른맥은 흔히 보이는 증상이다. 자궁외 임신의 문제점은 점점 자란 태아로 인해서 자궁외 임신이 된 부위(특히 자궁관)가 그것의 크기를 견디지 못해서 파열되는 것이다. 그렇게 되면 많은 양의 피가 한꺼번에 출혈이 되어 생명을 잃을 수도 있다. 대부분의 자궁외 임신은 현재의 발달된 진단방법으로 빠르고 정확하게 진단할 수 있다. 즉 직장과 자궁사이에 존재하는 복강의 일부분인 막힌주머니(복강 중에서 가장 낮은 부분에 존재하여 복강내의 출혈이나 기타의 액체는 이부분에 모인다)천자술로 0.5mm이상의 비응혈성 혈액의 흡입이 될 경우에 자궁외 임신의 파열로 인한 출혈임을 알 수가 있고, 초음파 영상법에 의한 진단, 최근의 면역 화학적 호르몬 측정법과 복강경 검사법, 또한 자궁내막 조직검사 등으로써 진단할 수 있다. |

| mumps in pregnancy | It has been stated, we believe erroneously, that mumps is dangerous when contracted during pregnancy. For example, vetter (infect med 14:730-733, 1997), citing a single 1980 article, writes: mumps infection during the first trimester of pregnancy can increase the rate of spontaneous abortion. Congenital anomalies associated with mumps infection during pregnancy include endocardial fibroelastosis; imperforate anus; spina bifida; and auditory, optic, and urogenital deformities. However, shepard in his authorative catalog of teratogenic agents (j hopkins u press, 8th edition, 1995) does not consider that mumps merits inclusion as a proven or even possible teratogenic agent, that is as an agent capable of causing a spontaneous abortion (a miscarriage) or causing congenital malformations (the baby is born with birth defects). Furthermore, mumps does not even warrant mention in smith's recognizable patterns of human malformation (by kl jones, wb saunders co., 5th edition, 1997), a key standard text on dysmorphology (the study of malformations). Based also on our review of the facts available at this time (oct, 1997), we conclude that there is insufficient evidence to label mumps as a particular hazard in pregnancy. (12 Dec 1998) |

| mumps immunization | The standard MMR vaccine is given to prevent measles, mumps and rubella (german measles). The mmr vaccine is now given in two dosages. The first should be given at12-15 months of age. The second vaccination should be given at 4-6 years (or, alternatively, 11-12 years) of age. most colleges require proof of a second measles or mmr vaccination prior to entrance. Most children should receive mmr vaccinations. Exceptions may include children born with an inability to fight off infection, some children with cancer, on treatment with radiation or drugs for cancer, on long term steroids (cortisone). People with severe allergic reactions to eggs or the drug neomycin should probably avoid the mmr vaccine. Pregnant women should wait until after delivery before being immunised with mmr. People with HIV or aids should normally receive mmr vaccine. Measles, mumps, and rubella vaccines may be administered as individual shots, if necessary, or as a measles-rubella combination.
