| 설명 | 치아의 조직이 세균에 의해 용해내지 파괴되어 점차 통증을 나타내다가 치아를 잃게 되는 하나의 세균성 병이라고 볼 수 있다. 우리나라에서는 영구치의 충치 이환율이 약 80%로, 한 사람이 2~3개의 충치를 갖고 있는 셈이며, 구강 보건상태는 도시, 농촌지역, 해안지역 순으로 좋다. 원인은 치태(dental plaque)로서, 이것은 점액이나 탈락한 잇몸의 세포들, 그리고 세균 등으로 이루어져 있으며, 점착성이 강하여 치아 표면에 잘 부착되어 양치질이나 칫솔질로는 쉽게 제거되지 않는다. 치태에는 그물모양의 구조내에 막대균과 알균이 섞여 있는데, 특히 내산성 사슬알균과 젖산균이 많다. 현재 가장 유력한 충치의 원인균으로 밝혀지고 있는 이 사슬알균은 설탕을 합성하는 능력을 가지고 있으며, 덱스트란(dextran)을 형성한다고 생각되는데, 이 덱스트란은 물에 녹지 않으며 끈적거리는 점착성이 있어 치태가 치아표면에 잘 붙어 있게 한다. 이 사슬알균은 설탕으로부터 유기산과 치태를 형성한다. 치태 중에는 사슬알균이 생산하는 산이 정체되어 있기 때문에, 이 산에 의해 치아 표면의 단단한 석회질을 녹인다. 대부분이 인산칼슘으로 구성되어 있는 치아의 표면은 산에 의해 칼슘이 분해되어 치아 표면이 거칠어지면서 충치가 진행된다. 이와 같이 세균과 이 세균이 생산하는 화학적 물질에 의해서 충치가 생긴다는 가설을 세균-화학설이라고 한다. 충치의 발생원인에는 이 세균-화학설 이외에도 미생물이 치아표면의 유기질 부위에 먼저 부착되어 산을 만들어 파괴시킨다는 단백용해설도 있으며, 또한 산에 의하지 않는 치아의 파괴, 음식물이 원인이 된다고 주장하는 학설도 있다. 설탕은 충치를 가장 잘 유발시키는 물질로, 섭취 빈도에 따라 충치가 증가한다는 것이 증명되고 있다. 비타민은 충치 요인에 그리 중요하지 않은 것으로 보이며 음식물 내에 존재하는 칼슘과 인 또한 중요하지 않다고 한다. 유전적 소인은 중요한 역할을 하리라 생각되지만 증명하기가 어렵다. |

| 설명 | 레이저를 이용한 수술방법. 레이저 광선은 일반적인 전구나 촛불과는 달리 빛이 퍼지지 않고 일정한 방향으로 진행하므로 집광렌즈를 사용하여 광선을 모으면 초점 부위에서 단위면적이 높은 에너지를 얻을 수 있다. 이 에너지가 수분으로 구성된 생물체에 작용하면 순간적으로 수분이 증발하므로 조직이 기화 소멸하게 되는데, 이러한 원리를 수술에 이용하는 것이다. 레이저는 수술칼 대신 조직을 자를 수 있고 조직을 증발시키기도 한다. 레이저수술의 장점은 출혈이 없는 수술이 가능하므로 병터를 정확하게 없앨 수 있고, 구강-후두 등 기능 보존이 필요한 미세수술이 가능하며, 수술 후 염증이 가벼워 통증이 적고 흉터가 적으며 입원기간이 짧거나 필요하지 않을 수도 있다는 점이다. 단점은 수술칼이나 전기메스보다 절개속도가 느리고, 레이저광선이 수술부위 이외의 곳에 닿지 않도록 주의해야 하며, 전신마취를 하는 경우에는 특수한 마취튜브를 사용해야 한다는 점이다. |

| 설명 | 심장동맥이란 심장에 혈액을 공급하는 동맥을 이르는 말로 심장역시 근육이어서 혈액을 공급받아야 수축을 할 수가 있다. 심장동맥이 좁아질 경우에는 심장에 혈액의 공급이 없어져서 심장동맥병이라는 병을 일으킨다. 심장동맥 두름길수술(coronary artery bypass surgery)이란 심장동맥병에서 좁아진 혈관부위 때문에 생기는 혈액공급이 적어진 부위에 혈액공급을 원활하게 해주는 수술방법으로 다른 부위의 혈관이나 인공적인 물질을 이용해서 좁아진 혈관부위의 앞, 뒤를 연결하여 혈류가 좁아진 부분을 지나지 않고 새로 연결된 부분을 지나게 하여 혈류를 증가시키는 방법이다. |

| 설명 | 암의 4대 치료법은 외과적 수술요법, 방사선 치료법, 항암 화학요법, 면역요법 등을 말하며 이중 수술요법과 방사선 요법은 국소적 요법으로서 암세포가 원발장기(처음 암이 발생한 장기)나 국소 림프절까지 국한되어 있는 제 1, 2기 암의 치료에 사용된다. 화학요법과 면역요법은 전신요법으로서 제 3, 4기 암에 주로 사용되며, 1, 2기 암의 치료 후 눈에 보이지 않게 남아 있을 수 있는 잔류암세포나 미세전이 암세포의 완전파괴, 사멸을 위해 사용된다. 백혈병, 림프종과 같이 화학요법제에 잘 듣는 혈액암, 림프종과 고환종과 같이 방사선 치료에 잘 듣는 암을 제외한 대부분의 고형암(solid tumor)인 위창자관암, 간암, 이자암, 유방암, 갑상샘암, 폐암, 흑색종, 연조직암, 뼈육종, 침샘암 등은 모두 수술요법으로 치료하여야 한다. 암수술의 기본원칙: 암수술의 3가지 기본요건은 안전성, 근치성, 기능보존성이며 암수술시에는 암병터를 되도록 조심해서 적게 만지면서 수술하고 암병터를 공급하는 동정맥과 림프관을 먼저 결찰하여 암병터가 퍼지는 것을 방지한다. 암수술의 목표는 근치적 절제수술(radical surgery)이다. 그러나 이것이 불가능한 환자에서는 출혈, 폐색, 천공 등의 합병증이 발생했을때 구급목적으로 혹은 지속적 동통이 있을때 생활내용의 질적 개선을 위하여 고식적 수술(palliative surgery)을 한다. 그리고 암의 정확한 진단과 진행도의 결정을 위한 진단적 수술(diagnostic surgery)이 있다. |
