| 설명 | 어머니에서 생산된 태아의 적혈구에 대한 항체가 태반을 건너와서 태아의 적혈구와 결합하여서 생기는 용혈성빈혈을 이르는 말. 즉 신생아나 태아의 적혈구의 항체가 어머니의 몸에서 만들어져서 이것이 태반을 통해서 태아에게 넘어가면 태아의 적혈구와 결합을 하고 이 항체와 결합한 적혈구는 파괴 되어서 결국 빈혈이 생긴 것을 태아적모구증이라고 한다. 이것은 Rh 적모구증과 ABO 적모구증로 나눌 수가 있다. 1) Rh 적모구증적혈구의 표면에는 면역 반응을 일으킬 수 있는 여러 가지 항원이 있다. 그중 D라는 항원의 유무에 따라서 혈액형을 Rh(+) 혹은 Rh(-)로 나눈다. Rh(+) 혈액형의 경우에는 적혈구 표면에 D라는 물질이 존재하고 Rh(-)의 혈액형의 경우에는 D라는 항원이 적혈구의 표면에 존재하지 않는다. 만약 어머니가 Rh(-)이고 임신된 태아가 Rh(+)의 경우에 정상적으로 태아의 적혈구는 태반을 통하여 어머니의 혈액속으로 들어갈 수가 없지만 분만도중에 태반의 작은 결손이나 상처를 통하여 모체의 순환 속에 들어가게 되면 Rh 음성인 모체에 처음으로 D라는 항원에 노출이 되고 이 항원을 외부의 물질로 인지를 해서 그것에 대한 항체를 만든다. 그리고 그 다음번 임신에서 만약 Rh(+)를 가진 아이를 임신하게 되면 이 항체가 태반을 통해서 태아에게 건너가서 태아의 적혈구와 결합을 하여 적혈구의 파괴를 유도한다. 2) ABO 적모구증적혈구표면에 존재하는 여러 가지 항원 중에서 A, B항원이 존재한다. 이것으로 혈액형을 정하는 방법이 ABO형 혈액형이다. ABO 적모구증은 어머니와 태아사이의 이 ABO 혈액형의 부적합으로 인해서 생기는 것으로 이미 정상적으로 존재하는 항체가 태반을 통해서 태아에게 건너갔을 경우에 생긴다. 즉 어머니가 O형의 혈액형을 가졌을 경우에 어머니는 정상적으로 A, B의 두 가지 항원에 대해서 모두 항체를 가지고 있다. 그런데 태아가 A형인 경우에 어머니에 존재하는 A에 대한 항체가 태반을 통해서 건너왔을 경우에 이것과 태아의 적혈구에 부착이 되고 항체가 부착이 된 적혈구는 역시 파괴가 된다. 대부분의 경우 어머니가 O형, 아기가 A형, 혹은 B형일 때 일어난다. ABO적아구증의 증세는 Rh 적모증보다 가벼우나 훨씬 더 빈번이 일어난다. 그러나 Rh 적모구증은 첫번째 아이에서 적게 생기고 대부분의 경우 두번째 아이에서 많이 생기는데 비하여 ABO 적모구증은 약 50%가 첫 아기에서 볼 수 있다. |
