| 설명 | 기관지가 과민하여 보통의 자극에도 기관지가 경련성으로 수축되고 점막이 부으며 점액이 분비되고 내강이 좁아져 숨 쉬기가 매우 곤란해지는 병. 대개 새벽녘에 발작적으로 호흡곤란이 오고 기침이 나며 가래가 나온다. 집 안의 먼지 속에 들어 있는 진드기, 꽃가루, 곰팡이, 동물의 털, 비듬 따위에 대한 알레르기 반응이 주원인이며, 자율신경의 기능이상이나 내분비 조절의 이상에도 원인이 있다. 발작은 넓은 범위에 걸친 중-소 기관지의 평활근 수축, 점막부종에 의한 협착과 기관지 강내의 가래에 의하여 야기된다. 천식과 가장 관계가 깊은 소인으로 알레르기 체질이 있다. 이는 원래 알레르겐의 작용에 의하여 항체를 만들기 쉬운 체질인데, 그러한 체질을 가진 사람에게는 각종 발작의 유인, 예컨대 감염-내분비 조절장애 (사춘기나 갱년기에서 볼 수 있다)-과로-정신적 스트레스 등 외에 꽃가루-실내외 먼지-곰팡이 등 알레르겐의 접촉에 의하여 발작이 오기 쉽다. 증세의 전형적인 특징은 기침-재채기 등의 전구증상에 이어 급속히 호흡이 곤란해지며, 호흡을 하면 그르렁그르렁하는 소리가 나온다. 그런 발작이 멎으면 소량의 찐득거리는 가래가 나오고 호흡이 차차 편해지면서 그렁거림도 진정되어 발작전의 평상시 상태로 돌아간다. 발작의 강약은 환자에 따라서 매우 다르며, 또 빈도나 지속시간도 가지가지이다. 발작은 야간 또는 새벽에 일어나는 경우가 많고, 계절적으로는 봄과 가을의 환절기에 많다. 진단은 특징 있는 발작상태 때문에 비교적 쉬운 편인데, 발작시에는 갖는 특징적인 의학적 소견으로는 양측 폐야 전반에 걸친 마른거품소리가 들리며, 가래에서는 호산구-샤르코-레이덴의 결정 등을 볼 수 있다. 치료법에는 대증요법과 근본요법이 있는데, 대증요법으로는 아드레날린이나 에페드린 등 교감신경자극제, 테오필린 계통의 약제 및 부신겉질 스테로이드 약제가 주로 쓰이며, 보조적으로 항히스타민제-진해제-거담제-진정제 등이 사용된다. 근본적 요법은 체질개선요법이라고도 불리는 것으로 알레르겐에 대한 탈민감요법이 있다. |

| 설명 | 천식이란 여러 가지 자극에 대해서 기관지가 과민한 반응으로 생기는 기관지의 가역적인 폐쇄를 의미한다. 즉 정상인에게서는 반응이 없는 자극에 대해서 기관지의 폐쇄가 생기고 그 자극이 없을 경우에는 기관지의 폐쇄가 소실하는 질환을 말한다. 천식의 원인은 주로 제1형 과민감반응에 의한다. 제1형 과민감반응이란 외계의 물질에 대한 면역 반응이 지나치게 항진되어 있는 병적인 상태를 말한다. 면역이란 외부물질에 대한 방어를 목적으로 하는 세포들의 활동을 의미하며 여기에는 항체의 형성, 세포들의 탐식작용(외부에서 들어온 물질을 면역기능을 하는 세포가 잡아먹는 것) 등이 포함된다. 항체의 형성은 외부로부터 들어온 물질을 우선 대식세포 등이 포식을 하고 이것에 대한 정보를 T-세포에게 전달하고 T-세포는 이 정보를 처리하여 집약된 정보를 B-세포에게 전달하며 이 세포로 하여금 그 물질에 대한 항체를 생성하게 한다. 항체는 외부의 물질과 정확히 결합하여 외부 물질의 활성을 없애는 물질이다. 이렇게 형성된 항체 중에서 IgE라는 것이 있는데 이것은 비만세포라는 세포의 표면에 부착되어 있다. 만약 이 IgE에 부착될 수 있는, 즉 이미 항체(IgE)가 형성된(이렇게 되기위해서는 그 물질이 이전에 한 번 몸속에 들어왔어야 한다) 물질이 다시 체내에 들어오게 되면 IgE가 부착된 비만세포에서 히스타민과 여러 가지 다른 물질이 나오게 되며, 이 물질들은 다른 염증에 관계하는 세포들을 모으게 되고, 기관지분비물의 분비를 증가시키고 또 기관지에 있는 근육세포를 직접, 간접으로 자극(교감신경계의 자극을 통해서 간접적으로 기관지의 근육세포를 자극)하여 수축하게 한다. 이로 인하여 기관지의 염증과 기관지의 수축이 유도된다. 기관지의 자극에 대한 과민성이란 이런 일련의 과정에 의해서 일어난다. IgE에 부착될 수 있는 물질이 사라진 경우에는 이런 일련의 과정이 일어나지 않게 되고 기관지의 염증이나 기관지의 수축같은 증상은 사라지게 된다. 천식을 가역성 기도폐쇄라고 부르는 이유가 바로 여기에 있다. |

| exercise asthma | <chest medicine> A disease process that is characterised by paradoxical narrowing of the bronchi (lung passageways) making breathing difficult. Treatment includes bronchodilators which are given orally or delivered as an aerosol (inhaled). Corticosteroids are reserved for more difficult cases.
