| 설명 | 급성 위염-창자염이 동시에 일어나는 증세. 각각 개별적으로 일어나기도 하지만 대개의 경우 위염-창자염이 동시에 발병되면서 하나의 병상을 나타내므로 일괄해서 이르는 말이다. 위와 창자의 내막에 생긴 급성 염증으로서 식욕부진, 메스꺼움, 설사, 복통, 쇠약감 등이 특징적으로 나타나며 여러 가지 원인, 즉 대장균, 황색포도알균, 살모넬라균종에 의한 식중독, 자극성 음식의 섭취, 스트레스, 공포감, 분노 등 심리적 원인으로 생길 수 있다. 급성 위창자염은 여러 종류의 식중독균에 의한 경우를 비롯하여 감기나 기관지염 등으로 위창자염은 여러 종류의 식중독균에 의한 경우를 비롯하여 감기나 기관지염 등으로 위창자염의 증세를 일으키는 경우도 많다. 그 밖에 소화가 잘 안되는 음식을 과식하거나 배를 차게 한 탓으로 일어나는 설사도 여기도 포함된다. 치료는 전신의 안정을 유지하고, 식사는 구역-설사-복통 등 급성증세가 없어질 때까지 되도록이면 제한하고, 소화가 잘 되는 것을 소량씩 점진적으로 먹는다. 수분은 특히 제한할 것 없이 적당하게 섭취하도록 한다. 세균성의 경우는 항생물질을 투여한다. |

| 설명 | 주로 소아에서 일어나고, 그 결과로 지속성 면역을 주는 전염성의 paramyxovirus 병. 흡기에 의해서 감염되는데, 가장 농후한 바이러스감염은 침샘내에 생기며, 특히 턱밑샘이나 혀밑샘보다도 귀밑샘이 더욱 심하다. 잠복기는 18~22일이다. 감염은 약 75%의 증례에서 증상을 나타낸다. 이들 중 귀밑샘염은 70%에서, 수막염은 10~15%(이들 중 반수에서 무증상성 수액세포 증가증이 동반된다)에서 일어난다. 부고환고환염이 사춘기 후의 남성에서 생기지만 그 후에 불임증을 속발하는 일은 드물다. 그 외의 증상은 보다 드물게 나타나는데 이자염, 관절염, 심근염, 난소염, 갑상샘염 및 젖샘염 등이 있다. 발열과 환부의 유통성 염증은 최초의 2일간에 가장 현저하고 다음 4~5일이 지나면 서서히 가라앉는다. 동시에 한 부위 이상을 침습하는 수도 있으며 가끔 속발적으로 침습되고 질환의 전 경과는 2~3주에 이른다. 부수되는 영속성의 신경학적 손상을 동반하는 수막뇌염은 드물다. |

| 설명 | 한국전쟁당시 국내에서 발견되면서 한때 한국형 출혈열(Korean hemorrhagic fever)이라고도 불리웠던 이 병은 갑자기 시작하는 오한, 두통, 근육통을 나타내고 이어서 3~5일 내에 전신 피부 및 공막출혈, 혈소판감소증, 단백뇨 및 신장 기능상실 등이 온다. 말기에는 쇼크 및 저체온이 될 수 있다. 처음 7~10일 간에 살아남은 환자들은 서서히 회복되나 단백뇨의 핍뇨는 수 주간 지속된다. 증세에 따라서 발열기, 저혈압기, 감뇨기, 이뇨기, 회복기로 비교적 특이하게 구분된다. Hantaan virus라고 불리는 이 병원체는 세포질에서 자라는 RNA 바이러스이며 Bunyaviridae과에 속하는 Hanta 바이러스속에 속하는 종(species)으로 한탄 바이러스 이외에 Puumula virus, Hill virus가 있다. 최근에 환자로부터 한탄바이러스와는 구분되는 Seoul virus가 분리, 동정됨에 따라서 야외형(Hantaan virus)과 도시형(Seoul virus)이 병원체가 서로 다르다는 점이 알려졌다. 한국을 비롯하여 만주, 시베리아 등지에서 유행하는 질환으로 알려졌으나 한탄바이러스의 발견을 계기로 혈청검사가 가능해지자 한탄바이러스 내지는 이와 항원구조가 유사한 바이러스로 생기는 감염증이 세계 각처에 분포되어 있다는 것이 판명되었다. 계절적인 발생은 특이하며, 국내에서는 봄과 가을에 두 번의 유행이 있다고 하지만 최근에는 가을의 유행이 크다. 특징적인 육안적 소견은 콩팥속질의 출혈, 오른심방출혈, 뇌하수체 전엽의 괴사이며, 이밖에 후복강 연조직의 심한 부종, 체강내로 누출된 체액 저류, 장막의 산재성 출혈, 돌창자의 점막출혈, 폐부종 내지는 폐출혈 등이다. 조직소견의 특징은 여러 장기의 출혈과 콩팥속질, 뇌하수체, 부신 등에 생기는 초점성 응고성 괴사와 각 장기의 광범성 단핵세포 침윤이며, 세정맥이 확장되고 울혈이 생기고 혈장과 적혈구가 혈관외로 누출되고 부위에 따라서는 초점성 모세혈관 파열이 나타나 있다. |
