| 설명 | 자궁의 가장 안쪽을 쌓고 있는 자궁내막의 특징을 갖추고 있는 조직이 정상적인 자궁내막이 존재하는 부위 이외의 장소, 즉 자궁근육 가운데나 난자를 생성하는 난소 등에 존재하는 것을 말한다. 즉, 자궁내막 상피세포와 자궁내막 버팀질을 동시에 갖는 경우를 말한다. 이것도 자궁의 내막과 거의 같은 성질을 가진 조직이므로 성적으로 성숙한 여성에게만 발생하며, 정상적을 자궁의 내막이 쇠퇴하는 폐경 후에는 저절로 사라진다. 또 자궁의 내막과 같이 성호르몬의 영향을 받아서 주기적으로 변화하며, 월경시에는 그 부분에서도 출혈이 일어난다. 때문에 자궁의 근육층에 자궁내막조직이 생기게 되면 근육층에 피가 고여서 하나의 덩어리를 형성하게 된다. 또 난소에서 발생한 자궁내막증에서는 난소에 피가 괴어 피로 차있는 덩어리를 만들게 된다. 이 자궁내막증이 있는 경우에는 월경통, 성교통 등의 통증을 유발하게 되고, 또 불임의 원인이 되는 수도 있다. 원인은 아직 잘 알려져 있지 않지만, 자궁내막의 절편이 월경혈과 함께 역류하거나 내막소파-자궁관통기-자궁수술 때 등에 자궁관을 통하여 난소나 복막면에 이식되어 증식한다고도 하고, 원생조직의 화생에 의한다고도 한다. 증세는 발생초기에는 특별히 자각되는 것이 없고, 또 있다고 해도 경미하며, 병터가 광범해짐에 따라 증세가 강하게 나타난다. 특히 드드러진 증세는 월경통으로서, 월경이 시작되기전부터 월경 제 1, 2일경이 가장 심하고, 그 후는 점차 가벼워진다. 자궁부정출혈이나 과다월경을 일으키는 수가 많고, 월경전기에 자궁이나 그 밖의 병터가 붓는다. 임신이 장애되어 불임이 되는 수가 많다. 근래 이 병이 증가하는 경향이 있어 주목되고 있다. 치료에는 호르몬요법 등의 보존방법과 수술요법이 있다. |
