| 설명 | 생물체가 만들어내는 독성과 항원성을 가진 물질. 독소를 적당한 동물에 주사하면 항체인 항독소가 생성되고, 이 항독소와 독소는 특이적으로 결합한다. 세균에 의한 단백분해산물로 맹독성을 발휘나는 것에 아민이 있으나, 이것에는 항원성이 없으므로 독소가 아니다. 일반적으로 중독증상을 발현하는데 일정한 시간이 걸리며, 동물에 따라 감수성이 다른 점이 독물과 다르다. 독소에는 생물체 안에 만들어져 남아 있다가 생물체가 파괴되었을 때 독성을 갖는 내독소(endotoxins)와 생물체가 주변으로 물질을 분비하는 외독소(exotoxins)가 있다. 독소를 내는 생물체에 따라 세균성 독소 외에 동물성 독소 및 식물성 독소가 있다. 동물성 독소는 대부분 독선에서 분비되며, 뱀-전갈-거미-벌 등이 대표적인 것이다. 식물성 독소로는 리신(ricin)-아브린(abrin)-로비닌(robinin)-크로틴(crotin) 등이 있다. 세균은 인체에 영향을 미쳐 디프테리아-파상풍-성홍열-가스괴저 등을 일으키고, 또 어떤 세균과 버섯류는 생장할 때 독소를 분비, 이것을 먹으면 보툴리누스 중독-맥각 중독 등을 일으킨다. 그 밖에 물고기에도 독성을 가진 것이 있다. 인체에는 독소가 들어왔을 때 그와 결합하여 독성을 해독시키는 항독소가 혈청-림프액 등에 생긴다. |

| 설명 | 디프테리아균이 기도의 상부에 달라붙어 독소를 발산하여 그 부위의 세포를 파괴하고 그 속에서 증식하면서 독소를 순환혈액 속으로 보내어 심장, 말초신경 등의 원격장기 및 조직의 기능을 저해하는 병을 이르는 말. 이 병을 일으키는 병원체의 정식 명칭은 Corynebacterium diphtheriae인데, 그 병원균이 생산하는 독소가 아주 치명적인 효과를 나타낸다. 사람의 감염에서도 그 독소의 독성이 가장 문제가 되는데 이 독소의 독성은 매우 강하여 독소 0.0001mg으로 제법 큰 쥐를 죽일 수 있다. 상기도로 침입하여 그곳에 하얀색의 거짓막(pseudomembrane)을 형성함으로써 호흡곤란을 일으킨다. 위막은 창백한 빛깔을 띤 두터운 조직인데 죽은 디프테리아균, 독소 때문에 죽은 세포와 그것에 대한 반응으로 제작된 섬유성 물질 등으로 이루어져 있다. 이 조직은 하부조직과 단단히 결부되어 있어서 억지로 박리하면 출혈한다. 이 위막이 인후두에서 기관지에까지 파급되어 호흡곤란을 일으킨다. 성기, 눈, 귀 등이 제1차 병소로 되는 수도 있다. 디프테리아 독소는 세포 속으로 침입하여 세포의 기능을 정지시키는데, 특히 심장근세포와 말초신경에서 장애를 일으키는 예가 많다. 잠복기는 1~7일이며 초기증상은 어느 부위에 위막이 형성되었는지 여부와 그 위막 형성이 어느 정도의 범위에 미치는가에 따라서 결정된다. 디프테리아가 생산하는 독소가 디프테리아가 일으키는 문제 중에서 가장 큰 원인을 차지하고 있으므로 그 독소의 생물학적 기능을 없애는 항체를 만들어서 투여하는 항혈청요법이 시행되어서 치명율은 3~20%로 낮아졌으나 후두를 침범당한 증례의 경우에는 아직도 치사율이 높다. 디프테리아에 대한 예방접종으로 유병자가 줄었으며 치사율도 10분의 1로 줄었다. |

| 설명 | 각종 영상진단법을 이용한 질병의 진단 및 치료를 이용한 질병의 진단 및 치료를 담당하는 학문이다. 현대의학에서 질병의 진단은 크게 3가지 방법으로 구분한다. 즉 문진-시진-촉진-타진-청진 등의 전통적 방법과 근래에 발전된 검사실 진단, 영상진단이 있다. 이중에서 영상진단은 인체 내부장기와 질병의 상태를 눈으로 직접 볼 수 있으므로, 질병의 진단에 있어서 가장 중요한 진단방법으로 널리 사용되고 있다. 또한 진단방사선과학은 이와 같은 질병의 진단뿐만 아니라 수술 또는 내과적 치료를 대신할 수 있는 치료방법을 개발하여 현재 치료의 한 분야도 담당하고 있다. 진단방사선과학은 취급하는 영상진단방법이 다중재적 방사선 분야, 핵의학 분야, 영상진단 분야 등으로 분류된다. 또한 영상진단분야도 인체의 중요한 장기별로 일반 방사선진단-신경계-심혈관계-소화기 방사선진단 등으로 세분화되고 있다. 현재 실용화된 초음파 진단-전산화 단층 촬영술-자기 공명 영상술 등은 컴퓨터와 전자공학의 발전이 의학에 적용된 영상진단분야이다. 진단방사선과학은 질환의 행태학적인 진단이 보다 정밀화됨으로써 생체조직에 대한 전자현미경적 검사와도 연관될 수 있게 될 것이다. 또한 영상진단의 수준을 형태학적 검사에서 한걸음 나아가 생리적-생화학적 현상의 영상화를 실현하고 이를 형태학적 진단과 접목시킴으로써 인체내부의 모든 상태를 천리안으로 투시하는 수준에 곧 도달하게 될 것으로 예상되고 있다. |

| diphtheria | <infectious disease> An acute infectious disease caused by toxigenic strains of Corynebacterium diphtheriae, acquired by contact with an infected person or a carrier of the disease, which is usually confined to the upper respiratory tract. It is characterised by the formation of a tough membrane (false membrane or pseudomembrane) attached firmly to the underlying tissue that will bleed if forcibly removed. In the most serious infections the membrane begins in the tonsillar (faucial) area on one tonsil and may spread to involve the other tonsil, uvula, soft palate and pharyngeal wall, from where it may extend to the larynx, trachea and bronchial tree and may cause bronchial obstruction and death by hypoxia. Diphtheria also occurs in a cutaneous form and may rarely involve the eyes, middle ear, buccal mucosa, genitalia and umbilical stump, usually secondarily.
