| 설명 | 1. 곧창자에 있는 대변덩어리를 항문을 통해 밖으로 배출시키는 생리적 작용. 곧창자벽 근육의 꿈틀운동이나 항문조임근의 억제 등이 종합적으로 작용하여 이루어진다. 식후 약 10시간만에 구불창자에 이른 큰창자 내용물은 수분이 흡수되어 분리가 되고 여기에 괸다. 한편, 음식물이 위에 들어가 자극을 주면 위큰창자반사가 일어나 큰창자에 강한 꿈틀운동을 일으키게 되고, 이때 대변덩어리는 잘룩창자에서부터 곧창자로 보내지며, 이것이 곧창자벽의 점막을 자극하면 감각신경(골반신경)을 통해 척수의 항문중추에 전달되어 반사적으로 항문조임근이 이완되면서 곧창자운동이 활발해진다. 또한, 이 감각은 동시에 대뇌에도 전달되어 변의를 느끼게 하고, 이에 따라 복압이 가해져 외항문조임근도 벌어지고 배변을 하게 된다. 변의가 생기더라도 어느 정도 의식적으로 배변을 저지할 수 있으나 창자꿈틀운동이 지나치게 강해지면 견디기 어렵게 된다. 또 엉치척수 또는 그보다 위쪽의 척수가 상해를 받으면 배변반사를 잃게 되어 억제할 수가 없게 되고 대변실금(자기 의지와는 관계없이 때와 장소를 가리지 않고 배설하는 것) 또는 배변이상을 일으키게 된다. 2. 불순물을 제거하는 것. |
