| 설명 | 심장에 지나치게 부담이 가서 심근이 두꺼워지고 심장이 커진 상태. 선천성 심장 기형, 심장 판막증, 고혈압증 따위의 각종 심장병에 의한 것과 운동선수나 육체노동자에게 나타나는 건강한 것이 있다. 비대(hypertrophy)의 기본 개념은 어떤 원인에 의하여 특정 장기가 세포 수의 변화와 상관없이, 즉 개개 세포의 용적 증가로 인하여 커지는 경우이다. 반면에 세포 수가 증가하여 커지는 경우는 증식(hyperplasia)이라 한다. 심장이 지속적으로 무리한 일을 하였을 경우에 생긴다. 생리적으로 운동 선수나 육체 노동자에게서는 골격근과 더불어 심근이 비대될 수 있다. 그러나 병적으로는 고혈압이나 폐동맥 협착증, 대동맥 협착증 등 심판막 질환이 있을 때 심장이 비대될 수 있다. 고혈압 환자에게는 대동맥의 높은 압력에 대항하여 심장이 비대되며 정상 무게인 350g에 비하여 700~800g까지 무거워질 수 있다. |

| 설명 | 고혈압이나 선천성 심질환, 기타 다른 질환의 결과에 의하지 않고 대부분 원인 불명으로 심근의 병변을 주로 하는 질환을 심근병증이라 한다. 임상적으로 심근병증은 확장성 심근병증, 비후성 심근병증, 제한성 심근병증로 구분한다. 확장성 심근병증란 여러 가지 원인에 의해서 또는 원인불명으로 모든 심실, 심방이 확장되고 이로 인해서 심장의 수축력이 떨어져 심장의 혈액을 순환시키는 펌프로서의 기능을 잃어버리는 것이다. 비후성 심근병증란 심장의 근육이 원인없이 두꺼워지는 것을 말한다. 심장의 근육이 너무 두꺼워지면 이것이 혈액이 나가야 할 통로를 막아 혈액의 순환을 방해하기도 한다. 제한성 심근병증란 심장의 근육사이사이에 이상한 물질이 침범해 들어와 이것으로 인해서 심장의 근육이 딱딱해지게 되어 심장근육의 이완을 어렵게 하는 것을 이르는 말이다. |

| 설명 | 심장이 정지했을 경우나, 물에 빠졌을 때와 같이 심장과 폐의 기능이 중지되었을 경우에 인공적으로 심장과 폐의 기능을 대신하여 주는 방법을 말한다. 심폐소생술의 기본은 ABC인데 이것은 airway(기도의 확보-기도의 이 물질을 제거하고 기도를 막고 있는 기타의 것을 없앤다), breathing(호흡의 유지-정상적인 폐의 기능을 유지하게 한다), circulation(혈액순환의 유지-심장의 기능을 정상적으로 만들어서 혈액의 순환을 정상적으로 유지하게 한다)의 앞자를 딴 것이다. 가장 기본적인 심폐소생술의 방법을 소개하면 다음과 같다. 1.기도의 확보(airway)-환자의 턱을 앞으로 당기고 머리는 뒤쪽으로 민다. 그리고는 손까락을 입속으로 잡아 넣어서 턱을 밀어서 입을 최대한 벌어지게 한다. 그리고 입속에 이물질이 있으면 제거한다(이렇게 하지 않으면 기도가 혀에 눌려서 막히게 된다). 2.호흡의 유지(breathing)-만약 환자가 숨을 쉬지 않을 경우에는 위의 자세에서 코를 한 손으로 막고 환자의 입에다가 입을 대고 힘차게 공기를 불어 넣는다. 이때 공기가 코나 다른 곳으로 새지 않게 주의를 한다. 그리고는 입을 떼어 불어 넣은 공기가 빠져 나오게 한다. 이때 환자의 가슴의 상하운동을 확인하여 적절히 시행이 되고 있는지 확인할 수가 있다. 호흡을 환자 자신이 스스로 할 수 있을 때까지 3-4초 간격으로 시행한다. 3.혈액순환의 유지(circulation)-만약 환자의 심장이 정지하였을 경우에는 환자를 반듯이 눕히고 시행자는 한 손을 명치 약간 위에 얹는다. 그리고는 나머지 한 손을 그 손위에 겹치고 환자의 가슴을 누른다. 이때 조심해야할 것은 시행자는 반드시 팔을 펴서 체중을 이용해서 수직으로 환자의 심장을 3~4cm정도 가슴이 들어가게 1분에 60번 정도의 횟수로 눌러야한다는 것이다. 이렇게 해서도 심장, 허파의 기능이 회복되지 않을 경우에는 여러 가지 약물이나 기구를 이용하여 심폐소생술을 시행한다. |
