| 설명 | 갑상샘속에 존재하는 조그만 호르몬 생성기관. 갑상샘속에 숨어 있으므로 눈에 잘 띄지 않는다. 그래서 흔히 갑상샘절제수술시에 같이 절제되어 부갑상샘호르몬감소가 나타나기도 한다. 부갑상샘호르몬을 생성한다. 이 호르몬은 뼈에서 혈액으로 칼슘(calcium)의 이동을 증가시키고, 위장관에서 칼슘의 흡수를 촉진시키며, 콩팥에서의 칼슘분비를 감소시키는 반면에 인산염(phosphate)의 분비를 촉진시켜, 결국은 혈중칼슘의 농도를 증가시키고 혈중인산염의 농도를 감소시킨다. 또한 이 호르몬은 비타민 D의 전환을 증가시킨다. 너무 지나친 부갑상샘호르몬의 분비가 있을 경우 골다공증(osteoporosis: 뼈에 구멍이 생기는 병적상태)과 다른 조직의 석회화를 볼 수 있다.  |

| barrett's oesophagus | A disorder in which the lining of the oesophagus undergoes cellular changes in response to chronic irritation and inflammation of reflux oesophagitis. This condition is more common in men than women. The patient with Barrett's oesophagus is at an increased risk of developing cancer of the oesophagus. Symptoms are similar to those of reflux oesophagitis and include heartburn, difficulty swallowing and pain relief with antiacid use or eating. The diagnosis of Barrett's is made by a biopsy of the oesophageal mucosa through a endoscope. Treatment includes control of reflux disease, weight reduction and avoidance of alcohol, tobacco, fatty foods and lying flat after eating. Acute, complete obstruction of the oesophagus occurs when food (usually meat) is lodged in the oesophageal stricture. Patients experience chest pain, and are unable to swallow saliva. Attempts to relieve the obstruction by inducing vomiting at home are usually unsuccessful. Patients with complete oesophageal obstruction can breathe, and are not at any risk of suffocation. | stricture of the oesophagus, chronic | A narrowing or closure of the normal opening of the swallowing tube leading to the stomach, usually caused by scarring from acid irritation. A common complication of chronic gastroesophageal reflux disease (gerd). Several procedures are available for stretching (dilating) the strictures without having to resort to surgery. One of the procedures involves placing a deflated balloon across the stricture at the time of endoscopy. The balloon is then inflated, thereby opening the narrowingcaused by the stricture. Another method involves inserting tapered dilators of different sizes through the mouth into the oesophagus to dilate the stricture. (12 Dec 1998) |
