| 설명 | 효소의 일종이며 여러 가지 동종효소(기능은 같지만 구조가 약간씩 다른 효소형태)로 신체의 여러 부위(특히 뼈와 간)에 정상적으로 많이 존재한다. 간내 쓸개관의 세포에 많이 존재하며, 뼈를 생성하는 세포인 뼈모세포(osteoblast)에 많이 존재한다. 그 외에도 여러 기관에 존재한다. 임상적 의의는 간과 뼈의 병 진단과 치료효과 판정에 도움을 준다. 간, 특히 간에서 만들어진 담즙을 샘창자까지 운반하는 쓸개길이 막힌 경우(쓸개관내에 종양이 생기거나 쓸개돌증으로 인해서 쓸개관이 막힌 경우)에는 쓸개내의 압력이 높아지고 간내 쓸개관의 세포가 깨져 그 속에 들어 있던 알칼리성 인산분해효소가 혈액속으로 방출되어 혈중 알칼리성 인산분해효소의 농도가 높아진다. 그리고 뼈의 생성이 많아지는 모든 경우(예를 들면 골절, 종양에 의한 뼈의 파괴시 복구를 위해서 뼈의 생성이 증가한다), 뼈모세포의 활동이 증가되고 뼈모세포의 효소인 알칼리성 인산분해효소의 활성도 역시 증가된다. |

| 설명 | 위점막의 염증성 병의 총칭. 그 본태는 아직 불명이지만, 진단법의 진보와 함께 위염의 개념은 변화하고 있다. 근래까지는 특수한 예를 제외하고는 내과적으로 관찰되었을 뿐이고 조직학적으로는 추구되지 않았으나 위내시경이나 위생검에 의한 위염진단의 진보와 함께 그 개념이 급속히 바뀌고 있다. 위염은 원발성 위염과 속발성 위염으로 구별한다. 속발성 위염은 급성위염에 뒤따라 일어나는 것도 있고, 다른 위장질환에 따른 증세로 오는 것, 또는 다른 병의 결과로 인하여 생긴다. 위염의 임상상은 막연하며 특유한 것이 없다. 따라서 원발성 위염은 위액검사와 대변잠혈반응 등을 참고로 진단하던 때를 지나, X선 검사를 하게 된 다음부터는 그 진단의 정확성이 매우 향상하였다. 근래 위내시경검사의 발달로 위점막을 직접 관찰할 수 있게 되어 진단의 정밀도는 더욱 높아졌으나 위액검사와 X선 검사를 포함하는 임상진단에서 위염이라고 진단이 내려진 것 중에는 위내시경검사 내지는 위카메라-위생검에서 기질적인 변화가 증명되지 않는 위증상군이 있다. 이들은 소화불량증-자극위 등으로 부르며, 또는 위신경증에도 속하는 것으로 보고 있다. 이와 같이 위염을 임상적으로 다루는 법은 지금까지 많은 변천을 보여 왔다. 위염의 진단은 조직학적으로 위 전체를 검사하는 일이기는 하나 그 실시는 한정되어 있다. 대개는 자각증-타각소견-위액검사-X선 검사-위내시경검사-위카메라검사-위생검 등으로 진단한다. 위염에 있어서 점막의 염증 중 가장 중요한 변화는 샘조직의 염증이다. 그러나 실제로는 샘조직뿐만 아니라 점막하층-근육틍-장막에 까지 염증이 미치고 있는 것이 있는데, 이와 같은 것들을 총괄해서 위염이라 한다. |
