| 설명 | 한국전쟁당시 국내에서 발견되면서 한때 한국형 출혈열(Korean hemorrhagic fever)이라고도 불리웠던 이 병은 갑자기 시작하는 오한, 두통, 근육통을 나타내고 이어서 3~5일 내에 전신 피부 및 공막출혈, 혈소판감소증, 단백뇨 및 신장 기능상실 등이 온다. 말기에는 쇼크 및 저체온이 될 수 있다. 처음 7~10일 간에 살아남은 환자들은 서서히 회복되나 단백뇨의 핍뇨는 수 주간 지속된다. 증세에 따라서 발열기, 저혈압기, 감뇨기, 이뇨기, 회복기로 비교적 특이하게 구분된다. Hantaan virus라고 불리는 이 병원체는 세포질에서 자라는 RNA 바이러스이며 Bunyaviridae과에 속하는 Hanta 바이러스속에 속하는 종(species)으로 한탄 바이러스 이외에 Puumula virus, Hill virus가 있다. 최근에 환자로부터 한탄바이러스와는 구분되는 Seoul virus가 분리, 동정됨에 따라서 야외형(Hantaan virus)과 도시형(Seoul virus)이 병원체가 서로 다르다는 점이 알려졌다. 한국을 비롯하여 만주, 시베리아 등지에서 유행하는 질환으로 알려졌으나 한탄바이러스의 발견을 계기로 혈청검사가 가능해지자 한탄바이러스 내지는 이와 항원구조가 유사한 바이러스로 생기는 감염증이 세계 각처에 분포되어 있다는 것이 판명되었다. 계절적인 발생은 특이하며, 국내에서는 봄과 가을에 두 번의 유행이 있다고 하지만 최근에는 가을의 유행이 크다. 특징적인 육안적 소견은 콩팥속질의 출혈, 오른심방출혈, 뇌하수체 전엽의 괴사이며, 이밖에 후복강 연조직의 심한 부종, 체강내로 누출된 체액 저류, 장막의 산재성 출혈, 돌창자의 점막출혈, 폐부종 내지는 폐출혈 등이다. 조직소견의 특징은 여러 장기의 출혈과 콩팥속질, 뇌하수체, 부신 등에 생기는 초점성 응고성 괴사와 각 장기의 광범성 단핵세포 침윤이며, 세정맥이 확장되고 울혈이 생기고 혈장과 적혈구가 혈관외로 누출되고 부위에 따라서는 초점성 모세혈관 파열이 나타나 있다. |

| 설명 | 토리콩팥염이란 이 토리의 염증을 주로 하는 콩팥병을 말한다. 이 토리콩팥염의 진단은 하지만 현미경적 관찰로 토리의 염증을 보고 진단하는 것이 아니라 임상증세로 진단하는 것이므로 다분히 임상적인 진단명이다. 즉 심하게 말하면 실제로 토리의 염증이 없는 경우에도 임상적 진단 기준에만 합당한 경우에는 토리콩팥염으로 진단이 내려질 수가 있다. 토리콩팥염은 급성과 만성의 두 가지로 나눌 수가 있다. 1.급성토리콩팥염-갑작스럽게 발생하는 혈뇨, 단백뇨, 그리고 콩팥의 기능부전으로 인해서 수분이나 전해질의 체내 축적, 불순물의 체내축적이 있는 병을 말한다. 이 급성 토리콩팥염을 일으키는 원인으로는 여러가지가 있지만 대표적인 것으로는 대부분 감염(사슬알균, 포도알균, 폐렴알균, 여러 바이러스 등)후에 초래되는 감염후 토리콩팥염이 있다. 여러 감염이 급성 토리콩팥염을 일으키나 특히 사슬알의 감염이 가장 전형적인 예라고 할 수 있다. 2.만성토리콩팥염-지속적인 요검사상의 이상, 즉 단백뇨, 혈뇨와 신장의 기능이 서서히 없어지는 질환군이다. 흔히 말기콩팥이라 부르며 거의 모든 콩팥의 병이 이 만성 토리콩팥염으로 진행된다. |

| 설명 | 바이러스에 의해 간에 생기는 급성염증. 급성간염이란 간염바이러스(A형-B형-비A비B형)에 의해서 간에 생기는 급성염증을 병명으로 이르는 말로, 이는 그 감염양식에 수혈 후에 발생하는 수혈후 간염과, 감염경로를 알 수 없는 산발성간염 및 집단으로 발생하는 유행선간염의 세가지 유형으로 나눌 수 있다. 수혈후 간염은 그 95%가 비A비B형간염이며 나머지가 B형 간염이다. 산발성 간염은 A형 간염과 B형 간염이 각각 30%를 이루고 나머지 40%는 비A비B간염이다. 집단으로 발생하는 유행성간염은 거의가 A형간염이지만 때로는 여기에 포함되지 않은 형의 간염일 경우도 있다. 급성간염의 증세는 먼저 몸이 나른해지고 온몸에 권태감이 찾아오며 조그마한 일에도 곧 피로를 느끼게 된다. 그리고 식욕부진-발열-구토증-복통-설사 등, 감기나 급성위장염에 걸렸을 때와 같은 증세 등이 나타난다. 뒤이어 황달증세를 보이는데, 이때는 초기의 증세가 약간 가벼워진 것처럼 느껴지는 것이 보통이다. 그러나 황달증세가 심해지고 초기의 증세들이 다시 진행되면 이때는 전격성간염이 될 위험이 있다. 간염 증세가 심하지 않았을 경우는 황달이 눈에 띄지 않은 경우도 있는데 이때는 진찰을 해도 감기나 급성위창자염으로 자칫 오진되기 쉽다. 또 A형간염은 열이 38~39℃까지 오르고 증세가 갑자기 나타나는 것이 특징이며 급성비A비B형간염은 증세가 비교적 가벼운 것이 특징이다. 급성B형간염의 증세는 A형간염과 급성비A비B형간염의 중간 정도인 것이 보통이다. |

| acute crescentic glomerulonephritis | <nephrology> A relatively uncommon (affecting 1 out of 10,000 people) form of acute glomerulonephritis that results in damage within the glomerulus of the kidney. There is rapid loss of kidney function with the formation of crescents on microscopic analysis (kidney biopsy). This disorder may result in acute glomerulonephritis or nephrotic syndrome, but ultimately results in renal failure and end-stage renal disease. Symptoms include smoky coloured urine (pyuria), decreased urine output, swelling and hypertension. Any conditions which can cause a vasculitis increase the risk of this disorder. It is often divided into types I and II, and sometimes III. | glomerulonephritis, membranous | A disease of the glomerulus manifested clinically by proteinuria, and sometimes by other features of the nephrotic syndrome. | membranoproliferative glomerulonephritis | <pathology> A kidney disorder which results in kidney dysfunction. Inflammation of the glomeruli result from an abnormal immune response and the deposition of antibodies within the kidney (glomerulus) ultrastructure. Membranoproliferative glomerulonephritis has been divided into two different types in the basis of where the antibodies are deposited in the glomerulus. MPGN type I, the more common type, deposits antibodies in the subendothelial layer of the basement membrane, whereas type II deposits antibodies in the bottom layer of the basement membrane. | membranous glomerulonephritis | <pathology> A kidney disease that occurs due to inflammation of the kidney glomerulus and its basement membrane. The exact cause is unknown but it appears to be related to the deposition of immune complexes in the basement membrane leading to thickening of the capillary walls. This disorder is a common cause of nephrotic syndrome an is usually how the disease manifests. Risk factors include primary renal disease, malaria, hepatitis B, lupus, syphilis, cancers and non-Hodgkin's lymphomas. Risks also include exposure to some medications such as gold compounds and penicillamine. Mercury, trimethadione and some skin-lightening creams have also been implicated. | mesangial proliferative glomerulonephritis | <nephrology, pathology> Inflammation of the kidney glomerulus (blood filtering portion of the kidney) due to the abnormal deposition of IgM antibody in the mesangium layer of the glomerular capillary.
