| 설명 | 복부 내장을 싸고 있는 장막. 복막은 아주 얇은 상피와 그 밑에 접착하는 결합조직의 층으로 형성되어 있다. 복막 전체는 하나로 되어 복강을 에워싸고 있다. 복강에는 복막액이라는 투명한 액체가 꼭 차 있어, 복부의 여러 내장은 상호간 또는 복벽에 대해 조금도 마찰을 일으키지 않고 움직일 수가 있다. 복벽의 내면을 덮고 있는 부분은 벽쪽복막이라 하고, 내장의 표면을 덮고 있는 부분은 내장쪽복막이라고 한다. 내장이 복막에서 어느 정도 멀리 떨러져 있는 곳에서는 복막이 벽쪽에서 내장쪽으로 이행하는 부분이 장간막을 형성하고 있다. 작은창자의 대부분을 차지하고 있는 빈창자와 돌창자에서는 장간막이 아주 잘 발달되어 있다. 큰창자에서도 가로창자와 구불창자는 기다란 장간막을 가지고 있다. 이들 부분은 다 복강 안에서 부드럽게 움직여, 상호 위치를 어느 정도 바꿀 수가 있다. 위도 장간막을 가지고 있으며, 이것을 위사이막이라고 한다. |

| 설명 | 자궁이란 태아가 수태되어서 분만전까지 발육하고 성장하는 공간이다. 자궁속에 병변이 있어 임신이 계속될 수 없거나 아니면 다른 이유로 임신되어 있는 태아를 제거하고자 할 경우에 사용되는 방법이다. 여기서 긁어내기 위하여는 우선 자궁의 입구에 해당하는 자궁목을 확장시켜야 한다. 여기에는 급속히 확장을 시도하는 법과 서서히 확장을 시도하는 2가지 방법이 있다. 자궁목을 급속히 확장할 때는 헤가르 목관확장기(Hegar's dilatator)를 사용한다. 이것은 작은 금속막대로 작은 크기부터 큰 크기까지 다양한 크기가 있어서 우선 작은 막대로 시작하여 점점 큰 크기의 막대를 자궁목에 넣어서 자궁목을 확장시킨다. 서서히 확장시킬 때는 Laminaria tent를 목관에 삽입하는 방법을 사용한다. Laminaria tent란 해초로 만든 작은 막대로 수분을 흡수하면 점점 늘어나는 성질이 있다. 이것을 자궁의 목에 넣으면 이것이 수분을 흡수하여 늘어나므로 천천히 자궁의 목이 늘어난다. 자궁목이 충분히 늘어나면 그 속으로 끝이 숟가락처럼 생긴 기구를 넣어서 자궁속의 병변이나 임신된 태아를 긁어내는데 여기에 사용되는 숟가락처럼 생긴 기구를 큐렛이라고 한다. 초기 임신중절 즉 유산과 같은 임신과 관련된 경우뿐만 아니라, 비임신 자궁의 자궁내막조직의 채취 및 제거를 위해서도 행해지는 수기이다. 이는 원칙적으로 마취하에 실시되는 것으로 자궁목관을 확장하고 기구로 자궁 내용물을 제거하고 큐렛으로 자궁내벽을 깨끗이 한다. 자궁천공이나 자궁목의 파열 등의 위험이 따르며, 수술후 감염 또는 출혈 등에 대한 주의가 필요하다. |

| 설명 | 결핵균에 의한 사람이나 동물의 감염으로서, 결절형성과 조직의 치즈괴사(caseation necrosis)가 특징이다. 주된 원인균은 Mycobacterium tuberculosis, M. bovis, M. avium, M. kansasii와 기타 분류되지 않은 비전형의 미코박테륨도 원인균이 된다. 전형적인 질병과는 임상적으로도 형태학적으로도 구별할 수 없는 비정형감염은, 사람에서 사람으로 직접 감염을 일으키는 것으로 생각되지 않는다. 결핵증은 그 증상의 발현에 있어 대단히 다양하며 만성화되기 쉽다. 모든 장기에 감염이 일어나지만, 사람에 있어서는 주로 폐가 침범되며, 그곳으로부터 혈관과 림프관을 통해 다른 장기에 감염된다. 증상은 오후의 미열과 수면중의 가벼운 식은 땀 등과 함께 침범된 장기의 기능 저하에 따른 증상이 동반된다. 치료는 항결핵제의 장기간, 집중적인 투여이며 필요에 따라서는 수술을 하기도 한다. |

| 설명 | 미코박테륨속의 사람 결핵을 일으키는 균이다. 1882년 코흐(Koch)에 의해 발견되었다. 그램양성 산소성 막대균이며 0.3~0.6×2~4μm, 무아포, 항산성이다. 항산, 항알칼리, 항알코올, 소독제에 저항한다. 저항력과 번식력이 강하여 전염성이 높으나 건조, 열, 햇빛에는 약하다. 세포벽에는 다량의 지방질이 포함되어 있다. 니아신검사는 사람형 결핵균만 양성이며 이것은 감별에 도움이 된다. 감염은 비말감염이고 폐결핵이 많지만 혈행 속에 들어가면 모든 장기에 결핵을 일으킨다. 특유한 결핵결절을 형성한다. 결핵의 진단에는 트베르쿨린 반응이 사용되며 예방에는 비씨지(BCG) 접종이 실시된다. 면역은 세포성 면역이며 큰포식세포의 활성화로 인해 균의 증식이 억제된다. |
