| 설명 | 활동시에 근대약과 피로를 호소하다가 안정 후에는 회복되는 특징적인 증상을 호소하는 병으로, 근육과 신경접합부 사이 연결부분 이상으로 발생한다. 주로 젊은 여성에서 빈발하고, 눈주위 근육을 잘 침범한다. 대개 아침이나 휴식 후에는 호전되었다가 운동시 악화되는 피로현상을 보인다. 메커니즘은 아마 면역계의 이상으로 인해 발생하는 것으로 보이며, 치료 또한 면역을 억제하는 약을 다량 투여함으로써 호전되었다는 보고가 많다. 침범부위의 근육에 따라 여러 가지 종류가 있으며, 호흡근까지 침범하게 되면, 호흡기능상실로 사망한다. 신경-근육 접합부 중 접합뒤막의 아세틸콜린 수용체의 장애가 발생하는 자가면역병이다. 따라서 신경-근육 접합부에서 신경자극 전달이 차단되어 근육쇠약과 조기 피로가 온다. 환자의 85~90%에서 혈중에 아세틸콜린 수용체에 대한 항체가 있다. 전자현미경으로 관찰시 신경 근육 접합부 접합주름 감소 및 단순화를 보인다. 환자의 15~50%는 가슴샘종과 동반되고 나머지는 가슴샘과다형성과 잘 동반된다. 광학현미경상에서는 2형 근육섬유의 위축과 탈신경에 의한 작고 각진 근육섬유가 나타나는 것 이외에는 특별한 소견을 보이지 않는다. |

Muscles of the face or upper trunk are especially likely to be affected. In most and perhaps all cases due to the development of autoantibodies against the acetylcholine receptor in neuromuscular junctions.
