| 설명 | 전신 자가면역병의 대표적 병이다. 여성에게 압도적으로 많고, 10~39세까지가 70%를 차지한다. 원인은 잘 모르나 자가면역 조절기능에 장애가 있는 것으로 생각된다. 유전요인, 호르몬의 영향, 환경요소가 종합적으로 보조 T세포와 B세포를 활성화시켜서 여러 가지 자가항체들, 즉 천연 DNA, 이중가닥 DNA, 외가닥 DNA, RNA, 핵단백질 등과 같은 핵 항원에 대한 항체, 비핵항원에 대한 여러 가지 항체가 쏟아져 나온다. 이 중 이중가닥 DNA를 포함한 순환면역복합체가 콩팥에 침착하여 손상을 일으키는데 관여한다. DNA와 여러 가지 세포성분에 대한 자가면역질환으로 전콩팥기를 침범하여 염증을 일으킨다. 여러 자가항원에 대한 면역반응에 의한 항체의존성 면역반응과 면역복합체 반응으로 토리콩팥염, 혈관염, 관절염 및 빈혈을 동반한다. |

| 설명 | 산부인과 영역에서 가장 빈번한 응급 병으로, 그 발생 빈도는 매년 증가하는 추세이다. 자궁외 임신은 수정란이 정상적인 위치인 자궁몸통의 내강에 착상되지 않고 다른 곳, 즉 난소에서 나온 난자를 자궁까지 운반하는 자궁관, 난자를 생산하는 난소, 자궁을 지지하는 여러 인대, 복강, 자궁의 입구에 해당하는 자궁경부 등에 착상되는 임신을 말한다. 착상하는 부위에 따라 자궁관 임신, 자궁목관 임신, 난소 임신, 복막임신, 인대내 임신 등으로 나뉘며, 가장 흔한 자궁외 임신은 자궁관 임신이다. 자궁외 임신이 증가하는 이유로는 우선 불결한 성적 접촉에 의해 전파되는 급성 난관염의 증가를 생각할 수 있다. 실제 급성 난관염을 앓았던 여성은 그 다음 임신에서 자궁외 임신이 될 확률이 정상인에 비하여 7배나 높은 것으로 알려져 있다. 또한 자궁내 피임장치를 사용하는 여성에서도 정상인보다 높은 율의 자궁외 임신이 보고되었다. 자궁외 임신이 되면 반드시 다양한 형태의 하복부 동통을 나타낸다. 대부분 최종 월경일을 기준으로 4주쯤 후부터 부정기 질출혈을 보인다. 또 과반수의 환자에서 유방통이나 멀미 등이 나타나기도 하나, 체온 상승은 드물다. 그러나 빠른맥은 흔히 보이는 증상이다. 자궁외 임신의 문제점은 점점 자란 태아로 인해서 자궁외 임신이 된 부위(특히 자궁관)가 그것의 크기를 견디지 못해서 파열되는 것이다. 그렇게 되면 많은 양의 피가 한꺼번에 출혈이 되어 생명을 잃을 수도 있다. 대부분의 자궁외 임신은 현재의 발달된 진단방법으로 빠르고 정확하게 진단할 수 있다. 즉 직장과 자궁사이에 존재하는 복강의 일부분인 막힌주머니(복강 중에서 가장 낮은 부분에 존재하여 복강내의 출혈이나 기타의 액체는 이부분에 모인다)천자술로 0.5mm이상의 비응혈성 혈액의 흡입이 될 경우에 자궁외 임신의 파열로 인한 출혈임을 알 수가 있고, 초음파 영상법에 의한 진단, 최근의 면역 화학적 호르몬 측정법과 복강경 검사법, 또한 자궁내막 조직검사 등으로써 진단할 수 있다. |

| drug-induced lupus | <dermatology> An inflammatory autoimmune disorder, similar to lupus, that develops in response to the use of a particular medication. It is characterised by anti-histone antibodies. More benign than the usual disease, with less renal involvement. The syndrome clears after stopping the offending drug. Drugs that are known to cause this reaction include procainamide, isoniazid, sulphasalazine, hydralazine, methyldopa, phenytoin, chlorpromazine and penicillamine. The arthritis, cardiac, pulmonary and systemic features may be present, but the kidney involvement (nephritis) and neurologic disease are rare. Symptoms generally resolve spontaneously after stopping the medication. Complications include myocarditis, pericarditis, thrombocytopenic purpura and infections. (18 Jul 2002) |

| lupus anticoagulants | Lupus anticoagulants are a common cause of a prolonged activated partial thromboplastin time (APTT) which is corrected by the addition of platelet -rich plasma (or phospholipids) and not by platelet-poor plasma. The methods of choice for detection of lupus anticoagulants are the kaolin clotting time using the rabbit brain neutralisation procedure and the dilute Russell viper venom test (dRVVT). Current data suggest that lupus anticoagulants and antibodies to negatively charged phospholipids (cardiolipin, phosphatidylserine, phosphatidic acid and phosphatidylinositol) are risk factors for arterial and venous thrombosis and for recurrent abortions in populations of patients which are distinct but overlapping. Drug-induced lupus anticoagulants are also associated with increased risk of thrombosis. Current testing for lupus anticoagulants and phospholipid antibodies (cardiolipin plus phosphatidylserine) may be useful for assessing risk in patients with systemic lupus erythematosus. Some data indicate that lupus anticoagulants assays are more reliable predictors of thrombosis, foetal loss and thrombocytopenia than are cardiolipin antibody (ACA) assays. Although the technology is in many ways more reliable, the rush to describe associations of lupus anticoagulants with various disorders has been much less scientifically unseemly than has been that of ACAs with their bewildering variety of clinical associations. | lupus coagulation inhibitor | An antiphospholipid antibody found in association with systemic lupus erythematosus (lupus erythematosus, systemic), antiphospholipid syndrome, and in a variety of other diseases as well as in healthy individuals. In vitro, the antibody interferes with the conversion of prothrombin to thrombin and prolongs the partial thromboplastin time. In vivo, it exerts a procoagulant effect resulting in thrombosis mainly in the larger veins and arteries. It further causes obstetrical complications, including foetal death and spontaneous abortion, as well as a variety of haematologic and neurologic complications. (12 Dec 1998) |
