| 설명 | 자궁이란 태아가 수태되어서 분만전까지 발육하고 성장하는 공간이다. 자궁속에 병변이 있어 임신이 계속될 수 없거나 아니면 다른 이유로 임신되어 있는 태아를 제거하고자 할 경우에 사용되는 방법이다. 여기서 긁어내기 위하여는 우선 자궁의 입구에 해당하는 자궁목을 확장시켜야 한다. 여기에는 급속히 확장을 시도하는 법과 서서히 확장을 시도하는 2가지 방법이 있다. 자궁목을 급속히 확장할 때는 헤가르 목관확장기(Hegar's dilatator)를 사용한다. 이것은 작은 금속막대로 작은 크기부터 큰 크기까지 다양한 크기가 있어서 우선 작은 막대로 시작하여 점점 큰 크기의 막대를 자궁목에 넣어서 자궁목을 확장시킨다. 서서히 확장시킬 때는 Laminaria tent를 목관에 삽입하는 방법을 사용한다. Laminaria tent란 해초로 만든 작은 막대로 수분을 흡수하면 점점 늘어나는 성질이 있다. 이것을 자궁의 목에 넣으면 이것이 수분을 흡수하여 늘어나므로 천천히 자궁의 목이 늘어난다. 자궁목이 충분히 늘어나면 그 속으로 끝이 숟가락처럼 생긴 기구를 넣어서 자궁속의 병변이나 임신된 태아를 긁어내는데 여기에 사용되는 숟가락처럼 생긴 기구를 큐렛이라고 한다. 초기 임신중절 즉 유산과 같은 임신과 관련된 경우뿐만 아니라, 비임신 자궁의 자궁내막조직의 채취 및 제거를 위해서도 행해지는 수기이다. 이는 원칙적으로 마취하에 실시되는 것으로 자궁목관을 확장하고 기구로 자궁 내용물을 제거하고 큐렛으로 자궁내벽을 깨끗이 한다. 자궁천공이나 자궁목의 파열 등의 위험이 따르며, 수술후 감염 또는 출혈 등에 대한 주의가 필요하다. |

| 설명 | 치아의 조직이 세균에 의해 용해내지 파괴되어 점차 통증을 나타내다가 치아를 잃게 되는 하나의 세균성 병이라고 볼 수 있다. 우리나라에서는 영구치의 충치 이환율이 약 80%로, 한 사람이 2~3개의 충치를 갖고 있는 셈이며, 구강 보건상태는 도시, 농촌지역, 해안지역 순으로 좋다. 원인은 치태(dental plaque)로서, 이것은 점액이나 탈락한 잇몸의 세포들, 그리고 세균 등으로 이루어져 있으며, 점착성이 강하여 치아 표면에 잘 부착되어 양치질이나 칫솔질로는 쉽게 제거되지 않는다. 치태에는 그물모양의 구조내에 막대균과 알균이 섞여 있는데, 특히 내산성 사슬알균과 젖산균이 많다. 현재 가장 유력한 충치의 원인균으로 밝혀지고 있는 이 사슬알균은 설탕을 합성하는 능력을 가지고 있으며, 덱스트란(dextran)을 형성한다고 생각되는데, 이 덱스트란은 물에 녹지 않으며 끈적거리는 점착성이 있어 치태가 치아표면에 잘 붙어 있게 한다. 이 사슬알균은 설탕으로부터 유기산과 치태를 형성한다. 치태 중에는 사슬알균이 생산하는 산이 정체되어 있기 때문에, 이 산에 의해 치아 표면의 단단한 석회질을 녹인다. 대부분이 인산칼슘으로 구성되어 있는 치아의 표면은 산에 의해 칼슘이 분해되어 치아 표면이 거칠어지면서 충치가 진행된다. 이와 같이 세균과 이 세균이 생산하는 화학적 물질에 의해서 충치가 생긴다는 가설을 세균-화학설이라고 한다. 충치의 발생원인에는 이 세균-화학설 이외에도 미생물이 치아표면의 유기질 부위에 먼저 부착되어 산을 만들어 파괴시킨다는 단백용해설도 있으며, 또한 산에 의하지 않는 치아의 파괴, 음식물이 원인이 된다고 주장하는 학설도 있다. 설탕은 충치를 가장 잘 유발시키는 물질로, 섭취 빈도에 따라 충치가 증가한다는 것이 증명되고 있다. 비타민은 충치 요인에 그리 중요하지 않은 것으로 보이며 음식물 내에 존재하는 칼슘과 인 또한 중요하지 않다고 한다. 유전적 소인은 중요한 역할을 하리라 생각되지만 증명하기가 어렵다. |

| 설명 | 1. 건강의 유지와 증진, 병의 조기 발견이나 예방 따위를 위해서 심신의 상태를 검사하는 일. 2. 의사나 치과의사가 자각적-타각적으로 건강한 사람에 대하여 그 건강 상태를 진찰하는 일. 건강을 유지하기 위해서는 질병의 진단과 치료가 필요하지만, 치료에 있어서도 자각증상이나 타각증상이 있고 나서 손을 쓰다 보면 이미 때가 늦을 경우가 많으므로, 발병 초기에 조기진단을 함으로써 건강생활을 적극적으로 설계하려는 데에 의의가 있는 것이다. 건강진단을 크게 나누면, 개인적 건강유지를 위한 것과, 국민 전체의 건강유지를 위하여 행정적으로 특정한 대상자에게 의무적으로 하는 것이 있는데, 후자의 경우를 ‘정기건강진단’이라고 한다. 근로보건관리규정에는 위생상 유해한 직장의 종업원은 연2회 이상의 건강진단을 필요로 하고 있으며, 그 밖의 직장에서는 연1회 이상으로 규정하고 있다. 학교보건법에서는 매년 학생과 교직원의 신체검사를 하게 되어 있고, 결핵예방법에서는 모든 국민은 연1회 이상 결핵에 관한 건강진단을 받도록 되어 있으며, 모자보건법에서는 임신부와 영유아에 대한 건강진단을 규정하고 있다. 이 밖에 접객업소에서도 전염병-피부병에 관한 건강진단을 받아야 한다는 규정이 업종별로 정해져 있다. |
