| 설명 | 어깨의 관절낭과 말초관절 연골 사이의 유합성 염증이다. 점진적인 어깨동통, 경직, 운동 제한 등이 특징이며, 오십대에 어깨에 오는 병이라 하여 오십어깨 라고도 한다. 최근에는 40대에도 발생한다. 처음에 다쳤을 때는 수 일 내에 통증은 사라지나 손상된 힘줄에 염증이 생기면 인체 스스로 치료하려고 칼슘을 조직 내로 흡수한다. 그러면 다른 성분은 칼슘을 지방층과 충격흡수 부위로 밀어내면 이곳에 물이 흡수되어 조직이 붓고, 혈행이 막히면서 악화되어 점점 어깨의 움직임이 줄어들고 이런 과정은 1~2년이 걸리면서 악화된다. 종래의 치료방법은 붙은 어깨에 관절움직임을 증가하려는 운동 치료와 강하게 당겨 분리하려는 치료를 하여 간혹 낫는 경우도 있었다. 최근의 연구는 초기에는 안정을 하고 조금씩 움직임을 증가하는 치료법이 우수하다고 보고하고 있다. |

| 설명 | 자궁이란 태아가 수태되어서 분만전까지 발육하고 성장하는 공간이다. 자궁속에 병변이 있어 임신이 계속될 수 없거나 아니면 다른 이유로 임신되어 있는 태아를 제거하고자 할 경우에 사용되는 방법이다. 여기서 긁어내기 위하여는 우선 자궁의 입구에 해당하는 자궁목을 확장시켜야 한다. 여기에는 급속히 확장을 시도하는 법과 서서히 확장을 시도하는 2가지 방법이 있다. 자궁목을 급속히 확장할 때는 헤가르 목관확장기(Hegar's dilatator)를 사용한다. 이것은 작은 금속막대로 작은 크기부터 큰 크기까지 다양한 크기가 있어서 우선 작은 막대로 시작하여 점점 큰 크기의 막대를 자궁목에 넣어서 자궁목을 확장시킨다. 서서히 확장시킬 때는 Laminaria tent를 목관에 삽입하는 방법을 사용한다. Laminaria tent란 해초로 만든 작은 막대로 수분을 흡수하면 점점 늘어나는 성질이 있다. 이것을 자궁의 목에 넣으면 이것이 수분을 흡수하여 늘어나므로 천천히 자궁의 목이 늘어난다. 자궁목이 충분히 늘어나면 그 속으로 끝이 숟가락처럼 생긴 기구를 넣어서 자궁속의 병변이나 임신된 태아를 긁어내는데 여기에 사용되는 숟가락처럼 생긴 기구를 큐렛이라고 한다. 초기 임신중절 즉 유산과 같은 임신과 관련된 경우뿐만 아니라, 비임신 자궁의 자궁내막조직의 채취 및 제거를 위해서도 행해지는 수기이다. 이는 원칙적으로 마취하에 실시되는 것으로 자궁목관을 확장하고 기구로 자궁 내용물을 제거하고 큐렛으로 자궁내벽을 깨끗이 한다. 자궁천공이나 자궁목의 파열 등의 위험이 따르며, 수술후 감염 또는 출혈 등에 대한 주의가 필요하다. |

| 설명 | 다발성 원발성골종양. 머리뼈-갈비뼈-복장뼈-척추뼈-골반 등에 잘 나타나고, 물렁물렁한 종괴를 형성하며, 뼈의 흡수가 일어나고, 40~60세 남자에게 많이 발생한다. 골수종 종양세포는 형질세포에서 유래한 것이어서 형질세포종이라고도 한다. 과거에는 형질세포성골수종 이외에는 다른 골수조혈요소에서 생기는 골수종이라고 생각했지만 현재는 부정되고 있다. 형질세포는 원래 면역글로불린을 생산하는 세포이며, 그것이 종양화한 다발 골수종 환자에서도 대부분 혈청 속에 면역 글로불린이 증가된 것을 볼 수 있다. 증가한 글로불린은 IgG나 IgA인 경우가 많지만 다른 형도 있다. 골수종 환자 약 50%는 오줌에서 벤스죤스단백질이 검출되는데, 이 단백질의 축적에 의해 요세관이 파괴되고, 콩팥경화가 일어난다. 골수종 환자에서는 혈청단백 이상으로 가끔 아밀로이드증이 나타난다. 뼈 X선 소견으로서는 도려낸 병터, 골융해상, 병적골절이 관찰된다. |
