| 설명 | 뇌와 척수는 뇌척수액이라는 맑은 액체로 둘러쌓여 있다. 이것은 뇌의 겉뿐만 아니라 뇌의 속도 존재하는데 뇌 속에 이 뇌척수액이 존재하는 공간을 뇌실(ventricle)이라고 한다. 이 뇌척수액은 주로 뇌실에 존재하는 맥락얼기(choroid plexus)에서 만들어져서 뇌실에 머물다가 뇌의 밖으로 빠져나와서 뇌와 척수의 겉을 순환하다가 뇌에 존재하는 특수한 정맥의 체계인 정맥동(sinus)로 흡수가 된다. 수두증이란 이 뇌척수액이 과다하게 뇌속에 존재하는 경우로 실의 확장을 특징으로 하는 질환이다. 수두증이 될 경우에 뇌실의 과도한 확장으로 인해서 뇌의 실질을 압박하여 뇌의 기능장애를 보이며 두개강속의 과도한 압력의 상승으로 인해서 여러 가지 악영향을 끼친다. 수두증의 원인은 뇌척수액의 과다생성이나 뇌척수액이 순환하는 통로의 패쇄에 의하는데 실제로 뇌척수액이 과다생성되는 것은 아주 드물고 대부분이 뇌척수액의 순환로의 패쇄에 의하는 경우가 대부분이다. 수두증은 교통성 수두증과 비교통성 수두증으로 구분하여 말하기도 하는데 비교통수두증은 뇌실내에 있는 뇌척수액의 통로의 패쇄가 있는 경우를 말하는 것으로 주로 선천적인 원인에 의한 것이 대부분이다. 즉 교통성 수두증에선 뇌실내의 압력만 상승을 하는 것이고 내실외부의 공간에선 압력의 상승이 있지 않다. 교통수두증이란 뇌와 척수를 쌓고 있는 뇌척수액의 공간의 병변에 의해서 뇌척수액의 흡수가 되지 않아서 생기는 것이다. 즉 뇌실과 뇌실외부의 공간압력이 같고 모두 상승을 하게 된다. 치료는 척수액 순환 경로의 폐쇄부위를 제거하고, 척수액 생산을 감소시키는 방법과 다른 곳으로 뇌척수액을 배출시키는 방법이 있다. 가장 대표적인 것으로는 뇌실과 복강을 튜브로 연결하여서 뇌실속의 뇌척수액이 복강으로 배출되게 하는 뇌실복막 지름길(ventriculoperitoneal shunt)이 있다. 치료를 하지 않을 경우에는 50%가 사망하지만, 수술을 하는 경우에는 80%가 생존하고, 생존자의 1/3은 신경학적으로 지능도 정상이 된다. |
