| 설명 | 위와 큰창자 사이의 길이 5m내외의 소화기관으로서 샘창자, 빈창자, 돌창자의 세 부분으로 구성됨. 1)샘창자: 소장의 제일 첫부분으로서 길이 25cm의 C자형모양. 상부, 내림, 가로, 오름의 네부위로 구성되며, 상부 부위를 제외하고는 모두 뒤복벽에 고정되어 있으며 내부에는 돌림주름이라는 점막 주름이 존재함. 위에서 넘어온 소화된 음식이 담즙, 이자액에서 분비되는 소화효소액)과 섞이는 장소이며 철이 흡수되는 장소임. 2)빈창자:작은창자의 두번째 부분으로서 근위부 2/5를 차지함. 공장은 회장에 비해 직경이 크고 장벽이 두꺼우며 혈관이 풍부하고 장간막이 투명하고 직행혈관이 긴 것이 특징임. 3)돌창자:작은창자의 마지막 부분으로서 원위부 3/5을 차지함. 특히 돌창자, 큰창자 이행부 부위에는 파이어판이라는 림프절이 존재하며, 이곳은 각종 질병의 중요한 발생 부위가 됨. 담즙염 및 비타민 B12가 흡수되는 장소임. |

| 설명 | 만성적이고 재발을 잘하는 창자의 염증을 특징으로 하는 병. 장의 벽은 안쪽에서부터 점막, 점막하조직, 근육층, 장막의 4개의 층으로 이루어져 있는데, 크론병은 이 모든 층의 염증을 동반한다. 장의 모든 부분에서 생길 수가 있지만 주로 막창자와 연결되는 큰창자의 말단부에 가장 많이 생긴다. 창자의 전층의 염증으로 인해서 장의 폐쇄나 괴양을 만들며 종종 천공된다. |

| 설명 | 소화관 중에서 위로부터 계속되는 가늘고 긴 관. 작은창자와 큰창자로 대별된다. 포유류에서는 작은창자는 다시 샘창자-빈창자-돌창자로 구별되고 큰 창자는 막창자-잘룩창자-곧창자-항문관으로 나뉜다. 사람의 작은창자는 길이가 6~7m이며, 위 속에서 암죽같이 된 음식물이 간에서 분비되는 쓸개즙, 이자에서 나오는 이자액, 창자벽에서 나오는 창자액 등의 소화액에 의해 소화시켜 흡수한다 작은창자 중 샘창자는 창자사이막이 없고, 복강 후벽에 유착되어 있으며, 전면만 복막에 덮여 있다. 빈창자와 돌창자는 창자사이막이 있다. 그 때문에 빈창자와 돌창자는 이동성이 매우 풍부하다. 작은창자의 내면은 전체에 걸쳐 융모라고 하는 비로드 모양의 점막소돌기가 나있다. 이 때문에 작은창자 내면의 흡수 면적은 넓어 그 총면적은 43m2에 달한다. 쓸개즙-이자액은 각각 온쓸개관과 이자관에 의하여 샘창자로 주입된다. 창자액은 샘창자샘과 창자샘에 의해 분비된다. 큰창자는 전체적으로 굵고 길이는 약 1.5m이다. 주로 수분을 흡수하며, 식물섬유 등 음식물의 일부가 소화된다. 돌창자에서 막창자로 이행하는 부분에 있는 돌막창자판막은 큰창자의 내용물이 작은창자로 역류하는 것을 막는 일을 한다. 잘룩창자는 오름-가로-내림창자의 3부분이 배안을 마치 액자처럼 둘러싸는 위치에 있으며, 오름과 내림창자는 복강후벽에 유착하고, 전면만이 복막으로 덮여 있다. 가로창자는 구불창자가 되어 소골반강으로 들어가 곧창자로 이어진다. 곧창자는 항문관을 거쳐 항문에 열리고 있다. 큰창자의 내면에는 작은창자와 같은 융모는 없다. 점막에는 점액을 분비하는 체세포가 많아 이것은 큰창자에서 점차 고형화하는 변괴을 원활하게 수송하는데 도움이 된다. 창자의 활동은 미주신경과 교감신경으로 된 자율신경에 의해 조절된다. |

| Crohn's disease | <disease, gastroenterology> An inflammatory disease of the gastrointestinal tract that seems to have both genetic and environmental causes, not well understood. The peak incidence of onset of this disease is between 15 and 25 years of age. Crohn's also occurs in later years between the ages of 55 and 60. Common symptoms include recurrent abdominal pains, fever, nausea, vomiting, weight loss and diarrhoea which is occasionally bloody. Complications include gastrointestinal bleeding, fistulas and anal fissures. Treatment includes anti-inflammatory drugs and corticosteroids. Surgery is successful in a select few. (27 Sep 1997) |
| disease, crohn's | A chronic inflammatory disease of the intestine. Named after Burrill Crohn who described the disease in 1932. It usually affects persons in their teens or early twenties. It tends to be a chronic, recurrent condition withperiods of remission and exacerbation. In the early stages, there are small scattered shallow crater-like areas (erosions) called apthous ulcers in the inner surface of the bowel. With time, deeper and larger ulcers develop, causing scarring and stiffness of the bowel. The bowel becomes increasingly narrowed, leading to obstruction. Deep ulcers can puncture holes in the bowel wall, leading to infection in the abdominal cavity (peritonitis)and in adjacent organs. When only the large intestine (colon) is involved, the condition is called Crohn's colitis. When only the small intestine is involved, the condition is called Crohn's enteritis. When only the end of the small intestine (the terminal ileum) is involved, it is termed terminal ileitis.When both the small intestine and the large intestine are involved, the condition is called Crohn's enterocolitis (or ileocolitis). Abdominal pain, diarrhoea, vomiting, fever, and weight loss can be symptoms. Crohn's disease can be associated with reddish tender skin nodules, and inflammation of the joints, spine, eyes, and liver. Diagnosis is by barium enema, barium X-ray of the small bowel, and colonoscopy. Treatment includes medications for inflammation, immune suppression, antibiotics, orsurgery.
