영문 oral administration 한글 경구복용
설명   
  약을 투여하는 방법에는 여러 가지가 있다. 크게 나누어 보면, 입을 거쳐 위창자계를 통해 넣는 방법과 위창자계를 통하지 않고 바로 혈액으로 넣는 방법이 있다. 위창자계를 통하지 않는 방법으로 가장 흔한 방법은 주사를 이용하는 방법이다. 하지만, 이외에 항문을 통해 넣는 좌약식방법과 혀밑에 넣는 혀밑투여법도 있다. 그러나 대부분의 약제는 경구복용을 하게 된다. 경구복용에 대한 약자는 p.o.(per oral)로 표기한다. 경구복용제의 단점은 복용한 약제가 위창자관계를 거치면서 사람마다 각기 다른 흡수정도와 대사정도를 거치게 되므로 일정한 농도유지가 어렵다는데 있다. 또한 경구복용제의 모양이 캡슐형인지, 혹은 가루형인지에 따라서도 같은 약이지만, 서로 다른 효과를 나타낼 수 있다.

    전체 암 발생 빈도에서 약 3∼4%를 차지하는 구강 암은 혀, 뺨, 치주조직, 또는 구인두의 어느 부위에도 침범할 수 있다. 이는 구강 내의 발생 위치에 따라 달라지기는 하지만 대개 여자보다는 남자에게서 약 2~6배 많이 발생하며 우리 나라에서는 1년에 약 천 명 내지 이 천명의 새로운 구강 암 환자가 발생하기 때문에 구강 암으로 투병 중인 환자만도 수 만 명에 이른다. 1996년에 시행한 후향성 연구에 의하면 전체 구강 암 환자의 평균 5년 생존율은 약 46%로 반 이상의 환자가 5년 이내에 사망하는 것으로 조사된바 있다. 구강 암의 유병률은 연령 증가와 관련이 있다. 40세 이후에 발생률이 극적으로 증가하기 시작하여 60세의 연령에서 일정한 유지 곡선을 형성한다. 어떤 요소가 구강 암 발생에 가장 큰 영향을 끼치는가에 대한 문헌 연구에서 담배와 술이 모든 구강 암의 75%를 야기해 이 질환에 가장 큰 영향을 미치는 것으로 나타났다. 흡연과 과음이 각각 구강 암의 위험성을 증가시키는 반면, 그들의 상합효과는 그 몇 배인 것으로 나타나서 골초에 알콜 중독이라면 정상인의 약 30배 이상 이환율이 높아진다. 구강 암의 흔한 전구 증상으로 알려진 백반증은 구강 점막에 흰 백태를 보이는 병변으로 이 질환이 암종으로 진행될 확률은 0.1%~36%의 매우 다양한 편차로 보고되고 있으나 일반적으로는 10증례 중 1증례가 이행되는 것으로 알려져 있다. 잘 적합되지 않는 틀니나 오래 사용하여 마모되고 날카로워진 금관 등에 의하여 협 점막이나 혀, 무치악 치조정 점막 등에 발생되는 궤양성 병변은 종국에는 구강 암으로 전이되기 쉬운 것으로 알려져 있어 주기적인 치과 검진이 필요하다. 구강 암의 위험 요소 중에는 유전적 요소도 있는 것으로 나타났다. 특정 집단 중에는 암을 가지고 있는 가족이 있으면 구강 암의 발생 위험성이 높은 것으로 알려져 있다. 흡연과 음주가 구강 암 발생에 가장 중요한 요인이기 때문에 중단하거나 최소한 줄이는 것이 매우 중요하다. 건강한 식생활은 구강 암을 예방할 수 있다. 신선한 과일이나 녹색 채소는 구강 암 발생률을 감소시킬 수 있고, 이는 비타민 A, C, E가 함유된 음식이 예방 효과를 갖는다는 사실을 뒷받침한다. 구강 점막을 데일 정도의 뜨거운 음식이나 자극성의 음식은 피하도록 하는 것이 좋다. HPV의 발현이나 P53 유전 인자의 변위와 같은 특정 유전학적 또는 초기 세포 변화가 구강 암과 관련이 있다는 발견은 최근 주목받고 있는 연구이다. 가까운 미래에는 구강 암의 위험성이 높은 환자의 구강 점막을 주기적으로 검사하거나 초기 세포 변화를 보이는 환자를 인지하는 것이 가능할 것이다. 구강 암의 조기 발견과 치료를 위한 유용한 기술의 개발은 이 분야에서 기대된다. 구강 암은 발생 초기에는 별다른 증상을 보이지 않고 서서히 진행되기 때문에 조기 발견이 무척 어렵다. 구강 악안면 영역의 암의 조기 발견은 여러 가지 임상적인 발견이 기초가 되는데 초기에는 대개 붉은 홍반증을 보이며 점막이 거칠어지고 약간 융기되나 이때는 염증성 병소와 비슷하여 감별이 곤란하다. 이후 진행되어 입안에 흰 막이 생기거나 입안이 헐어 궤양이 생기고 보름 이상 낫지 않고 지속되는 경우도 있다. 입안에 만성적으로 백색 병소가 있는 경우, 목에 멍울이 생기고 없어지지 않는 경우 구강 암이 의심되므로 좀 더 세밀한 진단을 받아보는 것이 권장되며 조기 진단 시 생존율은 현저하게 높아지게 된다. 구강 암 치료 방법은 대표적으로 외과적 수술, 방사선 요법, 항암 요법으로 이루어지며 단독으로 또는 복합적으로 시행된다. 최근에 면역 요법, 광 에너지 요법 등이 개발되고 있으나 아직 임상적으로 활발히 이용되고 있지는 못하다. 구강 악안면 부위의 종양의 절제 시에는 통상 악안면 부위의 결손으로 기능 장애와 안모 추형 등의 심미적인 손상과 이에 따른 정신 심리적 문제점을 야기할 수 있다. 구강 암 절제술 후 악골 및 연조직 결손이 있는 경우에 악골 재건을 단순히 복원이라는 개념을 떠나 심미적이고 저작 기능의 회복이라는 차원에서 재건하여 사회적으로 복귀할 수 있도록 하는 것이 중요하다.

cancer, oral Cancer of the mouth area. A sore in the mouth that does not heal can be a warning sign of oral cancer. A biopsy is the only to know whether as abnormal area in the oral cavity is cancer. Oral cancer is caused by tobacco (smoking and chewing) and alcohol use. Surgery to remove the tumour in the mouth is the usual treatment for patients with oral cancer.
