| 설명 | 수술을 하지 않고 적은 합병증으로 심장동맥의 반경을 늘일 수 있는 방법. 심장동맥(Coronary artery)이란, 심장의 혈액을 공급하는 심장에 존재하는 동맥을 말한다. 이런 심장동맥이 병적인 상태로 그 내강이 좁아지게 되면, 심장의 허혈이 발생하여 이른바 협심증을 유발하게 되고, 심하면 심장의 경색증이 발생하며 결국은 환자의 생명을 앗아간다. 따라서 이런 좁아진 심장동맥이 있으면 넓혀주어야 한다. 심장동맥을 넓혀주는 방법으로, 혈전에 의한 경우 약물을 이용하여 용해시킬 수도 있으나, 이미 오래된 경우는 약물로써 치료는 불가능하다. 이때 수술적 방법과 이 경피적경혈관심장동맥확장술을 시행한다. 수술적 방법은 직접 가슴을 열고 심장의 좁아진 혈관을 제거하고 다른 혈관으로 대치하는 동맥이식술이며, 경피적경혈관심장동맥확장술은 피부를 통하여 (경피적)피부밑의 동맥을 찾아, 동맥속을 지나서 (경혈관)심장동맥에 이르러, 동맥을 확장하는 방법을 말한다. |

| 설명 | 일반 단순 X-선 촬영시에는 이 담관의 모양은 볼 수가 없다. 그래서 특수한 방법을 이용하여 이 담관 모양의 변화나 담관 병변의 발견에 이용한다. 이런 방법을 담관조영술(cholangiography)이라고 대표적으로 다음과 같은 2가지 방법이 있다. ◈경유정맥담관조영술: intravenous cholangiography 항상 간에서 흡수하여 담도계로만 배설이 되고, 또 X-선 촬영시에 희게 나오는 물질을 정맥을 통해 주사한다. 그러면 이 물질은 간에서 흡수되어 담도계로 배설이 되면서 담관을 채우게 된다. 이렇게 하여 X-선 촬영을 하면 담관을 이 물질이 채우고 있으므로 담관이 X-선 촬영을 통해서 보이게 된다. 이렇게 담관의 모양을 촬영하는 방법을 intravenous cholangiography라고 한다. ◈피부경유경간쓸개관조영술: percutaneous transhepatic cholangiography 바늘로 피부에서 간을 거쳐 간관에 이르게 찌른다. 이후 그 바늘을 통해서 X-선 촬영시에 희게 나오는 물질(조영제라 한다)을 넣으면 그 조영제는 담관을 따라 십이지장으로 배설이 된다. 이때 X-선 촬영을 하면 담관을 이 물질이 채우고 있어서 담관의 모양을 알 수가 있다. 담관의 병변이나 진단을 알기위해서 시행하는 이런 방법을 percutaneous transhepatic cholangiography(약자로 PTC)라고 한다. |
