The nursing process is an interactive, problem-solving process used by the nurse as a systematic and individualized way to fulfill the goals of nursing care. It is a deliberate, organized and scientific approach requiring thought, knowledge and judgment. The five phases of the nursing process are Assessment, Nursing Diagnosis, Plan, Implementation and Evaluation (Kozier, Erb, Blais, Wilkinson, and Van Leuven, 1998).
