| 설명 | 인슐린이란 이자에서 분비되는 호르몬으로 주로 섭취된 영양소의 저장을 촉진시키는 역할을 하는 호르몬이다. 특히 흡수되어 혈액에 존재하는 당을 온몸의 세포속으로 이동시켜 저장하게 하며, 에너지원으로 사용하게 하는 역할도 한다. 즉 인슐린은 혈액속의 당을 세포속으로 이동시켜 그것을 에너지원으로 사용하게 하고 다른 지방, 단백질 등의 영양소도 역시 세포속에 저장시키는 역할을 한다. 만약 인슐린의 분비가 췌장에서 적어질 경우에는 세포들은 소화되고 흡수되어서 혈액 중에 존재하는 당분을 흡수하지 못하고, 이것들을 에너지원으로 이용하지 못하게 된다. 그러므로 혈액속의 당의 농도는 높아지게 된다. 또한 단백질이나 지방이 세포속에 저장이 되는 것도 억제가 된다. 당뇨병이란 이자에서의 인슐린 분비의 절대적, 또는 상대적 부족이나, 세포들에 대한 인슐린의 생물학적 효과 감소로 인하여 발생되는 고혈당(혈액속의 당의 농도가 높아지는 것) 상태 및 이에 수반되는 대사장애가 장기간 지속되는 상태로 특징지워지는 병이다. ◈당뇨병의 증상 당뇨병의 가장 전형적인 증상은 다뇨, 갈증, 체중감소, 쇠약감 등의 증상이다. 이러한 증상 이외에 당뇨병이 있는 경우에 여러 가지 합병증이 생기는데 이것도 당뇨병의 중요한 증상이 된다. 당뇨병의 합병증은 급성으로 대사의 이상에 의해 생기는 여러 가지 합병증과 만성적인 혈당의 상승으로 인해 작은 혈관의 파괴에 의해서 생기는 망막, 콩팥, 신경의 이상이 있다. ◈당뇨병의 진단 당뇨병의 진단은 비정상적으로 혈액속의 당의 농도가 높을 경우에 가능해진다. 비정상적으로 높은 혈액속의 당의 농도의 기준은 2회 이상 10~16시간 공복후에 혈액에서 측정한 당의 농도가 140mg/dl이상이거나, 식사후 2시간에 측정한 당의 농도가 200mg/dl이상인 경우에 진단이 된다. 이것은 혈액속의 당의 농도를 낮추는 인슐린의 부족이나 인슐린의 기능의 이상으로 인해서 혈액속의 당이 높은 것을 의미하는 것이다. ◈당뇨병의 분류 1.인슐린의존당뇨병: IDDM(Insulin-dependent diabetes mellitus)-IDDM은 당뇨병의 치료에 반드시 인슐린이 필요한 경우를 말한다. 주로 원인이 췌장에 있는 인슐린을 분비하는 세포의 파괴이며 이로 인해서 당뇨병의 치료제로 쓰이는 인슐린의 분비를 촉진하는 약물이 이 IDDM에서는 쓰일 수가 없고 오직 인슐린만이 치료제로 쓸 수가 있다. 전형적인 인슐린 의존형 당뇨병은 소아에서 흔히 발생하고 인슐린 분비세포의 파괴에 의해서 인슐린 분비능은 거의 없다. 2.인슐린 비의존성 당뇨병: NIDDM(Non-insulin-dependent Diabetes Mellitus)- NIDDM은 치료에 인슐린이 필요하지 않은 경우를 말한다. 이것은 인슐린분비세포의 파괴에 의해서 생기는 것이 아니고, 대신에 각 세포들의 인슐린에 대한 반응성이 떨어져서 당뇨병이 생기는 경우가 대부분이다. 그러므로 이자에서 인슐린의 분비를 증가시켜 주는 약물만으로 치료가 가능하며 인슐린은 아주 심한 경우에 제한적으로 사용이 된다. 인슐린 비의존형 당뇨병은 중년이나 노인에게서 흔히 발견되고 보통 증상이 뚜렷하지 않은 경우가 많으나, 어린이나 청년층에서도 발생하고, 증상이 꽤 심할 때도 있어서 다양한 편이다. 가족력이 강하고 젊은이에서 생기는 형을 연소형 인슐린 비의존형 당뇨병이라고 따로 분류하는 학자도 있으나, 일반적으로 받아들여진 용어는 아니다. 경제개발이 진행되면서, 비만증도 함께 증가하고 있으며, 남녀비도 여성이 늘어가는 추세를 보이고 있다. 이 당뇨병에서의 또 다른 특징은 강한 가족력이며, 이것은 유전적 원인을 강력히 시사한다. 당뇨병은 인슐린 분비의 결핍이나 또는 인슐린의 영향을 받는 세포의 인슐린에 대한 반응의 저하에 의해서 고혈당을 유발한다. 고혈당 그 자체는 그다지 문제가 되지 않지만 지속적인 고혈당으로 인해서 생기는 여러 가지 합병증이 당뇨병환자에게 있어서는 항상 문제점이 된다. 당뇨병의 합병증에는 다음과 같은 것이 있다. 1.급성대사성합병증-당뇨병에서 비교적 흔히, 그리고 급성으로 올 수 있는 합병증으로 심하면 의식의 장애, 혼수를 가져오는 것으로 저혈당증, 케톤산혈증, 고삼투압성 비케톤성 혼수가 여기에 속한다. 가)저혈당증(hypoglycemia)-치료 과정에서 생기는 합병증으로 급성대사성 합병증 중에서 가장 흔한 형태이다. 혈액속의 당의 농도가 떨어질 경우를 저혈당이라고 한다. 이것은 치료의 과잉에 의해서 과도한 인슐린의 작용으로 혈액속의 당이 거의 대부분 세포속으로 저장이 되어서 생긴다. 혈액 중의 당의 농도가 일정 수준 이하로 떨어질 경우에는 오로지 당만을 에너지원으로 이용하는 뇌에 공급될 당이 적어져서 뇌기능이 저하되면서 혼수 상태에 들어가게 된다. 이것의 치료는 우선 당뇨병환자에게 적당한 치료를 시행하여 혈액 중 당의 농도가 일정수준을 유지하게 하는 것이 가장 중요하며, 만약 저혈당증으로 인한 혼수가 왔을 경우에는 혈당을 높이는 조치(오렌지 쥬스, 쵸코렛 등을 먹인다)를 취하는 것으로 간단히 치료가 될 수 있다. 나)당뇨병성 케톤산혈증(diabetic ketoacidosis)-케톤체란 당이 세포들의 에너지원으로 사용될 수가 없는 경우(정상인에게서는 주로 장기간 단식을 할 경우에 만들어져 사용이 된다)에 사용이 되는 에너지원으로 간에서 생성이 된다. 당뇨병에서도 인슐린의 결핍에 의해서 세포들이 당을 섭취해서 에너지원으로 사용할 수가 없으므로 이 케톤체를 에너지원으로 이용하는 경우가 있다. 당뇨병성 케톤산혈증이란 케톤체가 에너지원으로 몸속에 많이 생기는 것을 말한다. 케톤체는 산성을 띠는 물질이므로 이 케톤체가 몸속에 많이 생성이 될 경우에는 혈액이 산성화되고 또 몸에 안좋은 일련의 과정이 일어난다. 주로 IDDM의 경우에 많이 생기며, 식욕감퇴, 오심, 구토, 복통, 의식장애 등의 증상이 생기며 심하면 사망에 이르기도 한다. 다)고삼투성 비케톤성 혼수(hyperosmolar nonketotic coma)-주로 NIDDM환자에게서 흔한 이것은 증상이나 발생기전이 당뇨병성 케톤산혈증과 유사하지만 케톤체가 검출되지 않는다는 특징이 있다. 주로 고령의 NIDDM환자에서 많이 생기며 심한 탈수가 생긴다. 혈당이 아주 높아지고 환자의 의식이 없는 경우가 흔하다. 2.만성합병증-당뇨병의 상태가 지속되면 많은 환자에게 있어서 체내 여러 조직의 변화가 일어난다. 이와 같은 변화는 대개 혈관의 변화에 기인한다. 당뇨병의 만성합병증은 원인인 혈관의 변화에 따라 대혈관의 장애와 소혈관의 장애로 나눌 수 있다. 가)대혈관의 장애-큰 혈관의 장애로 인한 합병증. 당뇨병이 지속될 경우에는 대혈관에 동맥경화가 잘 생긴다. 이로 인해서 뇌혈관질환이나 관상동맥질환의 위험성이 커진다. 나)소혈관의 장애-작은 세동맥의 병적변화로 인해서 생기는 만성합병증. 망막이나 신장, 신경의 작은 동맥에 이상이 생기므로 그곳에도 이상이 생긴다. 다)기타-혈당이 높아질 경우에는 세균이 번식을 많이 할 가능성이 높다. 즉 혈액속에 세균에게 영양을 제공할 많은 당이 존재하므로 정상인보다 훨씬 더 세균감염의 확률이 높다. 그리고 당뇨병이 있을 경우에는 피부의 특이한 병변이 생기는 경우가 있다. 당뇨병의 치료는 혈액속의 당의 농도를 정상으로 유지시키는 것이다. 혈당을 정상으로 유지하기 위한 방법으로는 다음과 같은 DEEDS라는 5가지의 수단이 있다. DEEDS는 영어의 앞자를 따서 만든 것으로 우선 처음의 D는 Diet 즉 식사의 조절을 말하는 것으로 식사의 과잉을 막아서 혈당을 조절한다는 것이다. 즉 혈당의 공급을 차단하여 혈당이 과도하게 높아지는 것을 막는다는 뜻을 가지고 있다. 두번째 나오는 E는 Education의 약자이다. 즉 환자의 교육을 통해서 환자자신이 혈당을 관리하도록 하여 스스로 살아갈 수가 있게 하는 것이다. 세번째 E는 Exercise의 약자로 운동을 통해서 혈당을 낮추도록 하는 것이다. 즉 식사 직후에 일정한 운동을 함으로써 식사로 인해서 혈당이 올라가는 것을 막자는 것이다. 네번째 나오는 D는 Drug의 약자로 약물에 의한 치료이다. 약물에는 인슐린과 경구혈당강하제의 두 종류로 나눈다. 인슐린은 당뇨병자체가 인슐린의 부족으로 생기는 병이므로 인슐린을 주사함으로써 부족분의 인슐린을 공급하는 것이다. 경구혈당강하제는 췌장의 세포를 자극하여 인슐린의 분비를 증가시켜서 당뇨병을 치료하는 약제로 Gilbenclamid, Chlorpropamide, Gliclazide 등이 있다. 마지막의 S는 Self-Monitoring의 약자로 스스로 자신의 혈당을 체크하여 식사, 운동, 약물의 조절이 알맞은 지를 알아보는 것이다. |

| Da Vinci, Leonardo | <person> Renaissance artists began to study the human body with great interest and sever their relations from stylised medical art, thus the progress of teaching anatomy was correlated to art. Artists even bought their pigments from the apothecaries and so became closer with the physicians. A Veronese anatomist, Marc Antonia Della Torre (1473-1506), desired to write a text of anatomy which would be the very best ever, and therefore he asked Leonardo da Vinci to do the drawing while he did the dissecting. This was very progressive until Della Torre passed away unexpectedly, and the plan did not reach maturity. Leonardo remained interested and then, fascinated with dissecting, he drew over 750 sketches in red chalk which were never put in print, in fact, they were not found for 200 years! Therefore a trained artist and sculptor with a pseudo-medical background, named Leonardo Da Vinci believed that a scientific knowledge of artistic anatomy could only be gained by dissecting the human body, which differed from the thinking of Galen and previous anatomists. This genius (architect, engineer, inventor) dissected some 30 (Da Vinci said more than 10) human bodies in the cathedral cellar of the mortuary of Santa Spirito, under the secrecy of candlelight and conceived about 750 sketches with notes in mirror writing. Leonardo injected the body blood vessels and cerebral ventricles with wax for preservation, an anatomical technique used today. His accurate drawings have been praised by anatomists, artists and medical illustrationists through the ages. He is the real Father of Anatomical Art. Because of human body sanctity by the church, and the papal decree forbidding human dissections, Da Vinci's manuscripts were hidden and remained unknown for over 200 years. Leonardo died at Amboise in 1519 at 67 years, while painting for the services of King Francis I of France. Can you imagine a person, working under the nose of the Pope and against his decree, stealing bodies of dead prisoners, dragging them into a church cellar, and studying their contents! The stench of stinking rotten corpses in a hot room without windows night after night, apprehensive of every footstep he heard, being in the presence of the dead, probably three to four cadavers at a time, and then getting rid of these corpses without being caught? These plans necessitated accomplices to execute them - indeed, devoted priests did so, against their religious belief. Although after his death his notebooks went to his friend Francesco Melzi and some to the Ambroisian Library in Milan, many drawings were stolen and got into the hands of other artists who claimed them as their own.
