| 설명 | 뇌에 전기 자극을 주어 인위적으로 경련을 유발함으로써 정신 병을 치료하는 방법. 전기쇼크요법(electroshock therapy)이라고도 한다. 1938년에 이탈리아의 체를레티와 비니에 의해 시작되었다. 작용기전은 확실치 않으나 뇌의 신진대사, 생화학 및 효소 등에 변화를 일으킨다는 생물학적, 생화학적 학설이 유력하다. 적응증으로 가장 효과적인 병은 우울증으로 특히 여성에서 갱년기에 호발하는 갱년기 우울증, 우울증과 조증(아주 기분이 좋은 상태가 지속되는 정신병)이 일정한 주기를 두고 반복하는 조울증에 잘 쓰이며, 80~90%에서 효과가 있다. |

| 설명 | 대뇌의 신경세포는 자발적인 활동을 하면서 전기적인 신호를 내보낸다. 이 신경세포의 전기적인 신호를 일정한 영역에서 집합적으로 얻어내서 기록한 것을 뇌파라 한다. 본래 뇌파는 시시각각으로 변화하는 뇌의 전기적 신호이지만, 연속적으로 기록하면 여러 형태의 파형으로 나타나기 때문에 뇌파라고 한다. 일반적으로 두피 위에서 기록한 두피뇌파외에 뇌속으로 전극을 넣어서 전기활동을 기록하는 심부뇌파가 있다. 1.주파수-가장 기본이 되는 뇌파는 8~13Hz의 주기를 가지는 알파파, 13~30Hz의 주기를 가지는 베타파, 4~7Hz의 주기를 가지는 데에타파, 0.5~3Hz의 주기를 가지는 람다파, 30Hz이상의 주기를 가지는 감마파로 분류된다. 정상 성인의 안정, 각성상태로 눈을 감고 있을 때의 기본파는 알파파가 주체를 이루고 있다. 베타파는 눈을 뜨고 있을 때나 긴장이 고조되었을 때 나타난다. 데에타파와 람다파는 서파라고 하며, 성인에게서 이런 파가 나타나면 이상상태로 진단된다. 어린이의 경우에는 정상시에도 데에타파가 출현한다. 2.진폭 또는 전위-기록상 50uV의 전위를 5~7mm가 되도록 조정한다. 150uV이상의 고전위를 가진 서파는 대뇌의 기능저하를 나타내고, 대뇌피질의 기능정지는 평탄파로 나타난다. 3.이상파-기본파와 그 파형, 진폭이 뚜렷히 다른 파를 이상파라고 한다. 이상파에는 극파(spike), 예파, 고전위파, 극서파결합 등이 있다. 이 파들은 간질 진단의 유력한 정보가 된다. 뇌파검사는 거의 모든 신경질환의 진단에 응용된다. 간질, 뇌종양, 뇌혈관장애, 두부외상, 염증, 변성질환, 선천기형 등의 진단에 응용된다. 뇌파는 뇌의 기능이상을 표현하는 것이므로, 뇌의 형태적이상을 나타내는 CT 등의 검사결과와 반드시 일치하지는 않는다는 점에 주의해야 한다. 즉 뇌파는 뇌의 기능이 정상적인가, 이상이 있는가, 만일 이상이 있다면 어디에 어느 정도의 장애가 있으며, 이 장애는 장차 어떻게 진전할까 등에 대해서 유력한 정보를 제공한다. 뇌파가 진단의 주요 무기가 되는 질환은 간질, 뇌의 국소적 기능이상, 의식장애, 특이적 이상파가 출현하는 질환 등이다. |
