| 설명 | 프리온은 전염성 해면뇌병증이라고 부르기도 하는데 사람에서 발병하는 크로이츠펠트-야콥병, Gerstmann-Straussler-Scheinker병, 쿠루, 치명적 가족불면증과 양과 염소의 면양떨림병(scrapie), 밍크의 전염성 뇌병증과 소의 해면뇌병증(광우병) 등을 포함한다. 이 병들은 대부분 뇌의 회색질의 해면모양 병터를 일으키며, 임상소견은 빨리 진행하는 치매이다. 이 병을 앓고 있는 숙주의 뇌조직을 인간이나 영장류에 주입하면 동일한 병이 발생한다. 프리온 단백(PrP)은 1982년 Stanley Prusiner의 연구실에서 면양떨림병을 전파시킬 수 있는 물질로 처음 발견되었고, 이 물질이 정상인 α-helix 아형(PrPc)에서 β-pleated sheet 아형(PrPsc 혹은 PrPres)으로 입체구조가 바뀌면 proteinase K 등에 분해되지 않는 감염력과 전염성을 갖는 물질이 된다. 이와 같이 PrPsc는 정상세포의 PrPc에서 형성되는데 PrPsc의 형성메커니즘은 아직 밝혀지지 않았다. 이런 입체구조의 변화는 자연히 발생하는 경우에는 속도가 매우 느리고, 가족력을 갖는 질환과 같이 유전자의 변이가 있는 경우에는 더 빠르게 생길 수 있다. 사람의 PrPc 유전자인 PRNP는 20번 염색체의 단완에 있다. 높은 보존강도에도 불구하고 한 종에서 유래된 감염물질이 다른 종에서도 정상 단백 유전자를 갖고 있는 동물에는 쉽게 감염된다. PRNP 유전자를 없앤 동물은 PrPsc의 감염에 내성이 있는 것이 관찰되어서 PrPsc의 감염에 PRNP 유전자가 관여하는 것을 알 수 있다. 신경조직에 PrPsc가 축적되면 프리온병이 발생하는 것으로 생각하지만 신경세포의 손상이 발생하는 기전은 아직도 규명되지 않았다. 프리온병은 대부분 산발성으로 발생하고 일부는 유전병이나 전염병과 비슷한 발병양상을 보인다. 유전병인 경우에는 PRNP 유전자의 다양한 변이가 관여하는데, 현재까지 가족력을 갖는 크로이츠펠트-야콥병과 치명적 가족불면증을 일으키는 유전자의 변화가 알려졌다. 프리온병의 전염경로는 PrPsc에 오염된 수술 기수, 뇌파검사시의 전극, 각막 등의 이식장기 또는 사체로부터 제조된 성장호르몬 등이다. |
