shunt, right to left <cardiology> A term used to describe the diversion of blood from the right side of the heart to the left. Long standing left to right shunts can lead to increased right sided pressures in the pulmonary circuit (pulmonary hypertension). Eventually the left to right shunt reverses and blood flows from the right to the left. Can be seen in large untreated ventricular septal defect or atrial septal defect.

Dickens shunt <biochemistry> A pathway of hexose oxidation in which glucose-6-phosphate undergoes two successive oxidations by NADP, the final one being an oxidative decarboxylation to form a pentose phosphate.
Diverges from this when glucose-6-phosphate is oxidized to ribose 5 phosphate by the enzyme glucose-6 phosphate dehydrogenase. This step reduces NADP to NADPH, generating a source of reducing power in cells for use in reductive biosyntheses.
In plants, part of the pathway functions in the formation of hexoses from carbon dioxide in photosynthesis. Also important as source of pentoses, for example for nucleic acid biosynthesis. acute renal failure <nephrology> A sudden decline in renal function may be triggered by a number of acute disease processes.
Examples include sepsis (infection), shock, trauma, kidney stones, kidney infection, drug toxicity (aspirin or lithium), poisons or toxins (drug abuse) or after injection with an iodinated contrast dye (adverse effect).
Chronic renal failure represents a slow decline in kidney function over time. Chronic renal failure may be caused by a number of disorders which include long-standing hypertension, diabetes, congestive heart failure, lupus or sickle cell anaemia. Both forms of renal failure result in a life-threatening metabolic derangement.
