heart attack The surface of a cholesterol plaque in a coronary artery can rupture, and when it does, a blood clot forms on the surface of the plaque. The clot blocks the flow of blood through the artery and results in a heart attack. (See the diagram below.) Heart muscle dies during a heart attack, and the loss of muscle is permanent. Blood and nutrients are not able to reach the part of the heart muscle below the blockage. The severity of the heart attack is a result primarily of the area of the heart that is affected.
