| 설명 | 암의 4대 치료법은 외과적 수술요법, 방사선 치료법, 항암 화학요법, 면역요법 등을 말하며 이중 수술요법과 방사선 요법은 국소적 요법으로서 암세포가 원발장기(처음 암이 발생한 장기)나 국소 림프절까지 국한되어 있는 제 1, 2기 암의 치료에 사용된다. 화학요법과 면역요법은 전신요법으로서 제 3, 4기 암에 주로 사용되며, 1, 2기 암의 치료 후 눈에 보이지 않게 남아 있을 수 있는 잔류암세포나 미세전이 암세포의 완전파괴, 사멸을 위해 사용된다. 백혈병, 림프종과 같이 화학요법제에 잘 듣는 혈액암, 림프종과 고환종과 같이 방사선 치료에 잘 듣는 암을 제외한 대부분의 고형암(solid tumor)인 위창자관암, 간암, 이자암, 유방암, 갑상샘암, 폐암, 흑색종, 연조직암, 뼈육종, 침샘암 등은 모두 수술요법으로 치료하여야 한다. 암수술의 기본원칙: 암수술의 3가지 기본요건은 안전성, 근치성, 기능보존성이며 암수술시에는 암병터를 되도록 조심해서 적게 만지면서 수술하고 암병터를 공급하는 동정맥과 림프관을 먼저 결찰하여 암병터가 퍼지는 것을 방지한다. 암수술의 목표는 근치적 절제수술(radical surgery)이다. 그러나 이것이 불가능한 환자에서는 출혈, 폐색, 천공 등의 합병증이 발생했을때 구급목적으로 혹은 지속적 동통이 있을때 생활내용의 질적 개선을 위하여 고식적 수술(palliative surgery)을 한다. 그리고 암의 정확한 진단과 진행도의 결정을 위한 진단적 수술(diagnostic surgery)이 있다. |

| 설명 | 유방은 지방조직과 본래의 목적인 젓을 생산하는 샘조직, 그리고 이 샘조직에서 생산한 젓을 밖에까지 내보내는 관조직, 그리고 이 샘조직과 관조직의 주위를 둘러싸고 있으면서 이것들의 모양을 유지해주는 간질로 이루어진다. 유방암이란 샘조직과 관조직에서 기원하는 종양을 말한다. 유방암을 발생시킬 수 있는 위험인자는 여러 가지가 밝혀져 있다. 그중에서 대표적인 것은 다음과 같다. 1.유전적 요인－가족 중에서 유방암에 걸린 사람이 있으면 유방암의 발생확률이 아주 높아진다. 2.에스트로겐에 장기적으로 많이 폭로되는 경우－에스트로겐은 여성호르몬으로 여성에서 정상적으로 생리의 주기에 따라 증감을 되풀이한다. 이 에스트로겐은 유방의 젖샘조직과 관조직의 증식과 성장에 관계한다. 그리고 이 에스트로겐에 많이 폭로되는 사람은 유방암의 확률이 높다. 외부에서 에스트로겐의 제제를 치료목적으로 투여할 경우에 유방암의 발생확률은 정상인에서 보다 현저하게 높아진다. 그리고 아이를 낳지 않고 평생을 사는 여자의 경우에 임신을 하고 아이를 낳은 여자에 비해서 에스트로겐에 많이 폭로가 되므로(임신시에는 비임신시보다 에스트로겐에 적게 폭로가 된다)유방암의 확률이 높다. 3.환경적 요인－지방질 섭취의 증가, 술의 섭취 등은 유방암의 확률을 높인다. 유방암의 증상으로 가장 많은 것은 아프지 않은 유방의 덩어리이다. 그리고 유두에서 분비물이 나오거나, 유방의 피부변화 등이 유방암의 흔한 증상이다. 치료는 유방의 절제술이 가장 효과적인 치료법으로 되어있다. 그리고 이것에 더하여 방사선 치료나 항암제요법도 효과가 있는 것으로 되어있다. 또 에스트로겐의 과잉과 관계가 있는 종양일 경우에는 에스트로겐의 효과를 차단하는 약물도 효과가 있다. |

| 설명 | 자궁의 목은 자궁이 질과 연결된 부위로 자궁의 입구에 해당하는 부위이다. 이곳의 상피세포에서 발생하는 암을 자궁목암이라고 한다. 자궁목암의 발생을 살펴보면 자궁목의 상피세포가 여러 가지 자극에 의해서 정상세포와 차이나는 암성변화를 한 세포가 생기게 된다. 이 암성변화를 한 세포는 처음에는 자궁목의 상피세포층의 일부를 이루고 있게 된다. 그러다가 점점 더 진행이 되면 암성세포가 자궁목 상피세포의 모든 층을 꽉채우게 된다. 그리고 계속 진행하면 상피세포의 맨 밑에 있는 바닥막을 뚫고 버팀조직으로 침윤해 들어가게 된다. 암성변화를 한 세포가 자궁의 상피층의 일부를 채우는 것을 자궁목형성이상(cervical dysplasia)라고 하고 암성변화를 한 세포가 자궁의 상피층을 모두 채워버려 정상적인 세포가 없는 경우를 상피내암종(carcinoma in situ)라고 한다. 그리고 바닥막을 뚫고 밑의 버팀조직으로 침범해 나가는 경우를 침윤암(invasive cancer)라고 한다. 자궁목암은 여러 명의 상대와 성관계를 가지는 여자, 그리고 첫 성경험의 나이가 어린 여자에게서 흔하고 또 사회적, 경제적 수준이 낮은 사람에게서 잘 걸리는 것으로 되어 있다. 이런 것으로 미루어 자궁목세포의 암성변화에는 성병과 같은 감염이 중요할 것으로 생각되고, 또 몇가지 바이러스의 감염이 자궁목암의 원인이라는 것이 밝혀져 있다. 자궁목암의 진단방법으로는 질세포검사, 콜포스코피, 생검(biopsy) 등이 있다. 세포검사란 조기진단 목적으로 가장 많이 사용되는 방법으로 솜봉으로 자궁목을를 문질러서 자궁목상피세포를 얻고 이것을 현미경으로 관찰하여 상피세포의 변화를 보는 것이다. 콜포스코피란 자궁의 목을 4~10배 정도 확대해서 보는 일종의 확대경검사으로 자궁목암으로 인한 자궁목의 병적변화를 관찰하여 진단하는 것이다. 생검란 진단을 위해서 직접 자궁목의 조직을 떼어 채취하여 현미경으로 관찰하여 진단하는 것으로 가장 정확한 진단법이다. 치료로는 병의 진행정도에 따라 다르겠지만 암의 진행이 거의 없어 형성이상이나 제자리암종 또는 아주 조금 침윤하였을 경우에는 수술적요법으로 자궁을 적출하는 것이 치료의 원칙이고 많은 진행을 보여 다른 조직으로 암의 침윤이 있을 경우에는 방사선 치료를 원칙으로 한다. |

| oral candidiasis | <gastroenterology, microbiology, oncology> Infection with a fungus of the genus Candida. It is usually a superficial infection of the moist cutaneous areas of the body and is generally caused by Candida albicans, it most commonly involves the skin (dermatocandidiasis), oral mucous membranes (oral candidiasis), respiratory tract (bronchocandidiasis) and vagina (vaginal candidiasis or thrush). Rarely there is a systemic infection or endocarditis. Oral candidiasis: describes a fungal (yeast) infection of the oral cavity due to Candida. It is common in infants, diabetics or those on chemotherapy and is well recognised in patients with HIV infection and AIDS. Oesophageal candidiasis: Infection of the oesophagus by the yeast-like fungus Candidal albicans. Usually occurs in the immunocompromised individual (AIDS or following chemotherapy). Oral candidiasis is a predisposing factor but oesophageal involvement can occur without evidence of infection in the oral cavity. Symptoms include difficulty swallowing, pain on swallowing and oral lesions. Diagnosis is made using endoscopy. Treatment is with antifungal agents such as ketoconazole or fluconazole.
