| 설명 | 고열과 발진이 주증세인 열성-급성의 법정전염병. 세계보건기구(WHO)의 보건규칙에 의한 국제 감시 전염병의 하나이기도 하다. 또한, 이 병의 유행은 전쟁과 관계가 깊어 전쟁티푸스 또는 기근열-교도소열 등의 별명이 있다. 그것은 이 병의 매개곤충인 ‘옷이’가 의류나 몸이 더러울 때 발생하기 쉬우므로 군대나 교도소, 전쟁터 등 환경이 나쁜 곳에서 크게 유행하게 되었기 때문이었다. 제1차 세계대전 당시 러시아에서 발생한 환자의 수는 2,500만 명이나 되었고, 영국과 기타 유럽에서도 전쟁-기근 때문에 이 병이 대유행했던 기록이 있으며, 일본에서도 제2차 세계대전 직후에 유행 했었다. 오늘날에는 아프리카-유럽-아시아의 일부지역에만 존재할 뿐이다. 병원체는 리케차 프로바제키(Rickettsia prowazeki)로 옷이에 기생증식하여 옷이의 분변과 함께 배설되면서 옷이가 흡혈하기 위해 피부에 준 상처나 사람이 손으로 긁어 생긴 상처를 통하여 감염된다. 잠복기간은 10~14일이며 급작스런 오한이나 함께 발열하여 3일 정도 경과되면 40℃ 전후의 고열이 나게 되고, 두통-관절통-결막충혈 등과 지름 2mm 안팎의 붉고 작은 출혈성 발진이 온몸에 많이 생긴다. 증세는 장티푸스와 비슷하고, 바일-펠릭스반응(Weil-Felix test)이라 하는 혈청반응에 의해 감별된다. 클로람페니콜이나 테트라사이클린계의 항생물질이 특효를 보여 치사율도 낮아졌다(20%). 예방으로는 살충제로 옷이를 구제하는 것이 가장 중요하며, 발진티푸스백신 접종도 유효하다. |

| 설명 | A형용혈사슬알균에 의한 법정 급성전염병이다. 5~10세의 소아에서 호발한다. 작은 침방울을 통하여 감염되며 잠복기인 2~4일을 지나, 초기증상은 고열, 인두점막, 편도가 현저하게 발적종대된다. 1~2일 후 비융합성의 홍색소구진이 전신에 출현한다. 입 언저리에는 발진이 없고 입술주위는 창백하며 딸기혀를 본다. 2~3일 후 발진이 사라진다. 병변부의 대부분은 표층이 박리되는데 특히 손과 발의 융합성 탈피가 특징이다. 현재는 과거에 비하여 훨씬 병이 가벼워져서 중이염, 바위, 화농림프절염도 드물다. 인두점막배양으로 균을 증명하여 진단한다. A군 사슬알균감염의 합병증으로는 면역복합체성콩팥염과 류마티스열이 된다. |

| 설명 | 1형 단순 헤르페스 바이러스(herpes simplex virus)의 감염에 의해서 생기는 피부병으로 주로입술에 병변을 가지는 병을 말한다. 입술포진(herpes labialis)와 동의어로 쓰인다. 즉 단순포진중 입주위에 생기는 것을 지정하는 말이다. 단순헤르페스(herpes simplex)란 보통 작은 물집이 여러 개 모여 생기는 피부병을 말한다. 잘 발생하는 곳은 입술, 외음부 등이며 주로 피부와 점막의 이행부에 나타난다. 물집은 쌀알만한 크기의 작은 물집 또는 고름을 포함하는 물집으로 몇 개가 무리를 지어 나타나는데, 며칠 지나면 물집의 막이 터지고, 피부의 표면이 진무르고 헐어서 작은 상처를 형성하여 노출된다. 그리고 딱지가 앉아 굳어진 다음 7~10일 가량 지나면 저절로 낫는다. 단순성 포진은 발생 부위에 따라서 입술포진과 음부포진으로 구분된다. 입술포진은 피로하거나 골프장 또는 해수욕장 등지에서 햇볕에 심하게 노출된 후에 유발되는 경우가 많다. 음부포진은 성교에 의하여 전염되는 경우도 있으며, 중증일 대에는 격심한 아픔을 수반하며 주위의 림프절의 종대를 초래하기도 한다. 직업상 접촉 기회가 많은 의사나 간호사의 경우는 손가락 끝에 발생할 때도 있다. 유아의 경우에는 고열과 인두통 등의 심한 증세가 나타나기도 하여 때로는 목숨까지 잃는다. |

| 설명 | 한국전쟁당시 국내에서 발견되면서 한때 한국형 출혈열(Korean hemorrhagic fever)이라고도 불리웠던 이 병은 갑자기 시작하는 오한, 두통, 근육통을 나타내고 이어서 3~5일 내에 전신 피부 및 공막출혈, 혈소판감소증, 단백뇨 및 신장 기능상실 등이 온다. 말기에는 쇼크 및 저체온이 될 수 있다. 처음 7~10일 간에 살아남은 환자들은 서서히 회복되나 단백뇨의 핍뇨는 수 주간 지속된다. 증세에 따라서 발열기, 저혈압기, 감뇨기, 이뇨기, 회복기로 비교적 특이하게 구분된다. Hantaan virus라고 불리는 이 병원체는 세포질에서 자라는 RNA 바이러스이며 Bunyaviridae과에 속하는 Hanta 바이러스속에 속하는 종(species)으로 한탄 바이러스 이외에 Puumula virus, Hill virus가 있다. 최근에 환자로부터 한탄바이러스와는 구분되는 Seoul virus가 분리, 동정됨에 따라서 야외형(Hantaan virus)과 도시형(Seoul virus)이 병원체가 서로 다르다는 점이 알려졌다. 한국을 비롯하여 만주, 시베리아 등지에서 유행하는 질환으로 알려졌으나 한탄바이러스의 발견을 계기로 혈청검사가 가능해지자 한탄바이러스 내지는 이와 항원구조가 유사한 바이러스로 생기는 감염증이 세계 각처에 분포되어 있다는 것이 판명되었다. 계절적인 발생은 특이하며, 국내에서는 봄과 가을에 두 번의 유행이 있다고 하지만 최근에는 가을의 유행이 크다. 특징적인 육안적 소견은 콩팥속질의 출혈, 오른심방출혈, 뇌하수체 전엽의 괴사이며, 이밖에 후복강 연조직의 심한 부종, 체강내로 누출된 체액 저류, 장막의 산재성 출혈, 돌창자의 점막출혈, 폐부종 내지는 폐출혈 등이다. 조직소견의 특징은 여러 장기의 출혈과 콩팥속질, 뇌하수체, 부신 등에 생기는 초점성 응고성 괴사와 각 장기의 광범성 단핵세포 침윤이며, 세정맥이 확장되고 울혈이 생기고 혈장과 적혈구가 혈관외로 누출되고 부위에 따라서는 초점성 모세혈관 파열이 나타나 있다. |

| oral candidiasis | <gastroenterology, microbiology, oncology> Infection with a fungus of the genus Candida. It is usually a superficial infection of the moist cutaneous areas of the body and is generally caused by Candida albicans, it most commonly involves the skin (dermatocandidiasis), oral mucous membranes (oral candidiasis), respiratory tract (bronchocandidiasis) and vagina (vaginal candidiasis or thrush). Rarely there is a systemic infection or endocarditis. Oral candidiasis: describes a fungal (yeast) infection of the oral cavity due to Candida. It is common in infants, diabetics or those on chemotherapy and is well recognised in patients with HIV infection and AIDS. Oesophageal candidiasis: Infection of the oesophagus by the yeast-like fungus Candidal albicans. Usually occurs in the immunocompromised individual (AIDS or following chemotherapy). Oral candidiasis is a predisposing factor but oesophageal involvement can occur without evidence of infection in the oral cavity. Symptoms include difficulty swallowing, pain on swallowing and oral lesions. Diagnosis is made using endoscopy. Treatment is with antifungal agents such as ketoconazole or fluconazole.
