| 설명 | 자궁이란 태아가 수태되어서 분만전까지 발육하고 성장하는 공간이다. 자궁속에 병변이 있어 임신이 계속될 수 없거나 아니면 다른 이유로 임신되어 있는 태아를 제거하고자 할 경우에 사용되는 방법이다. 여기서 긁어내기 위하여는 우선 자궁의 입구에 해당하는 자궁목을 확장시켜야 한다. 여기에는 급속히 확장을 시도하는 법과 서서히 확장을 시도하는 2가지 방법이 있다. 자궁목을 급속히 확장할 때는 헤가르 목관확장기(Hegar's dilatator)를 사용한다. 이것은 작은 금속막대로 작은 크기부터 큰 크기까지 다양한 크기가 있어서 우선 작은 막대로 시작하여 점점 큰 크기의 막대를 자궁목에 넣어서 자궁목을 확장시킨다. 서서히 확장시킬 때는 Laminaria tent를 목관에 삽입하는 방법을 사용한다. Laminaria tent란 해초로 만든 작은 막대로 수분을 흡수하면 점점 늘어나는 성질이 있다. 이것을 자궁의 목에 넣으면 이것이 수분을 흡수하여 늘어나므로 천천히 자궁의 목이 늘어난다. 자궁목이 충분히 늘어나면 그 속으로 끝이 숟가락처럼 생긴 기구를 넣어서 자궁속의 병변이나 임신된 태아를 긁어내는데 여기에 사용되는 숟가락처럼 생긴 기구를 큐렛이라고 한다. 초기 임신중절 즉 유산과 같은 임신과 관련된 경우뿐만 아니라, 비임신 자궁의 자궁내막조직의 채취 및 제거를 위해서도 행해지는 수기이다. 이는 원칙적으로 마취하에 실시되는 것으로 자궁목관을 확장하고 기구로 자궁 내용물을 제거하고 큐렛으로 자궁내벽을 깨끗이 한다. 자궁천공이나 자궁목의 파열 등의 위험이 따르며, 수술후 감염 또는 출혈 등에 대한 주의가 필요하다. |

| 설명 | 뇌의 기질적인(organic-:이 말은 기능적인(functional)에 반하는 말로써) 모든 검사를 시행하면 어떤 이상을 발견할 수 있다는 뜻이다. 바꾸어 말하면, 기능적인 이상에 의한 뇌증후군은 어떠한 검사로도 이상을 발견할 수 없으나 분명히 환자에게 이상증상이 나타났을 때 이를 묶어서 말한다. 이상에 의해 신경학적인 이상을 나타내는 일련의 병적현상을 모두 통틀어 말한다. 이 병은 흔히 보아 마치 정신병환자처럼 말을 횡설수설하고, 알아들을 수 없는 말을 하며, 때로는 다른 사람에게 공격적인 성향을 나타내기도 한다. 그리고 다른 사람과 도저히 교류를 할 수 없는 정서를 나타내기도 한다. 그러나, 이 병이 다른 정신병과 구별되는 특징적인 증상은 먼저, 의식의 혼탁이 동반되는 경우가 많고, 또한 그 증상의 정도가 변한다는 것이다. 즉, 아침에는 정상적인 행동을 하다가 오후가 되면, 의식이 흐려지면서 말을 횡설수설한다면, 이는 기질성뇌증후군일 가능성이 높다. |

| 설명 | 뇌란 머리뼈속의 공간인 두개강속에 존재하는 기관으로 인간의 사고, 운동, 감정 등 모든 인간의 활동과 인체의 변화를 관장하는 곳이다. 뇌는 앞뇌(forebrain), 중간뇌(midbrain), 마름뇌(hindbrain)의 3부분으로 나눌 수가 있다. 앞뇌는 다음과 같은 2개의 부분으로 다시 구분된다. 대뇌란 뇌의 가장 큰 부분을 차지하는 곳으로 두 개의 반구로 구성되어 있다. 사고, 운동, 성격, 기억 등의 고차원적인 기능을 행하는 곳이다. 사이뇌－대뇌의 속에 파묻혀 있는 곳으로 시상와 시상하부가 포함된다. 시상은 계란모양의 구조물로써 후각을 제외한 모든 감각신경의 중요한 통로가 된다. 시상하부는 시상의 하부에 존재하는 구조물로써 자율신경계, 호르몬, 식욕, 성욕 등의 조절에 중요한 역할을 하는 곳이다. 중간뇌－앞뇌 뒤에 나오는 마름뇌의 사이에 위치하여 이 두 곳을 연결하여 주는 역할을 하는 이곳은 운동신경과 감각신경의 중요한 통로가 많이 존재하는 곳이다. 마름뇌－다리뇌, 숨뇌, 소뇌의 3부분이 이곳에 속한다. 다리뇌와 숨뇌는 척수와 위의 뇌부분을 연결하는 곳으로 중간뇌와 마찬가지로 중요한 신경의 통로가 되는 곳이며, 대부분의 뇌신경(척수를 거치지 않고 뇌에서 직접 나오거나 들어오는 감각 또는 운동신경, 후각신경, 시신경 등이 그 예이다)의 기능에 중요한 역할을 한다. 소뇌은 평형감각을 담당하는 부분이다.  |

| 설명 | 뇌기능이 완전히 정지되어 회복 불능한 상태. 뇌의 기능에는 대뇌반구의 기능 이외에 뇌간의 기능도 포함된다. 뇌사는 인간의 죽음과 같은 뜻이 아니고, 보통은 뇌사 다음에 죽음이 온다. 심장이식 수술에는 가능한 한 신선한 심장이 필요하기 때문에 심장 제공자의 죽음을 뇌사로 규정하려는 움직임이 있었다. 1968년 8월 세계의사회 총회에서 채택된 장기이식에 관한 선언(시드니 선언)에서는 ① 심장 제공자의 죽음의 판정은 뇌파측정상의 뇌파의 정지(뇌사)로 결정해야 한다. ② 제공자의 죽음을 확인하는데 두명이상의 의사가 입회해야 하며, 뇌사의 결정에 참여한 의사는 이식수술에 관여해서는 안 된다고 했다. 그러나 뇌파측정만으로는 뇌줄기의 기능정지를 판정할 수 없고, 뇌사의 판정도 질병의 종류나 진도에 따라 기준이 달라지므로 판정기준의 검토가 필요하다. 뇌종양-뇌외상-혈행장애 등 뇌질환의 증례에서 보면 ① 깊은 혼수, ② 양쪽 동공의 확대 및 동공의 빛반사와 각막반사의 소실, ③ 호흡의 정지, ④ 뇌파의 평탄화, ⑤ 혈압의 급격한 저하와 그에 따른 저혈압 등 다섯 가지 조건이 6시간 후에도 동일한 상태에 있는 것 등 여섯가지 조건이 판정 기준이 되었었다. 현재는 가장 강력한 동통자극에 대해서도 전혀 아무런 감수성과 반응성을 나타내지 않는 24시간에 걸친 혼수로서, 자발운동 또는 자발호흡이 없고 유발반사가 소실되고 뇌의 전기활동이 없어지는 회복불가능한 의식소실이라고 정의하고 있다. |

| 설명 | 뇌종양이란 뇌와 뇌조직에서 생긴 종양을 지칭하는 말이다. 그러나 대개 넓은 의미로 사용할 경우에는 머리뼈속의 공간인 두개강속에 생기는 모든 종양을 이르는 말로 사용된다. 뇌종양은 한정된 공간인 두개강에서 발생하므로 종양이 그다지 크지 않아도 정상적인 조직을 압박하게 되고, 두개강내의 압력을 높인다. 이런 특징에 의해서 뇌종양의 증상은 다른 종양과 달리, 종양 그 자체의 증상보다도 두개내압상승과 정상조직의 압박에 의한 증상이 많다. 두개내압(뇌압)의 상승에 의한 증상으로는 두통, 구토등이 있으며, 지속적인 뇌압상승에 의해서 유두부종(papilledema)이 관찰되기도 한다. 그리고 정상적인 뇌조직의 압박과 종양이 생긴 부위의 기능의 결합에 뇌의 그 부분에 해당하는 기능의 상실을 보게된다. |
