| 설명 | 적혈구의 표면에는 면역 반응을 일으킬 수가 있는 여러 가지 항원이 있다. 그중 D라는 항원의 유무에 따라서 혈액형을 Rh(+) 혹은 Rh(－)로 나눈다. Rh(+) 혈액형의 경우에는 적혈구 표면에 D라는 물질이 존재하고 Rh(－)의 혈액형의 경우에는 D라는 항원이 적혈구의 표면에 존재하지 않는다. 만약 어머니가 Rh(－)이고 임신된 태아가 Rh(+)의 경우에 정상적으로 태아의 적혈구는 태반을 통하여 어머니의 혈액속으로 들어갈 수가 없지만 분만도중에 태반의 작은 결손이나 상처를 통하여 모체의 순환 속에 들어가게 되면 Rh 음성인 모체가 처음으로 D라는 항원에 노출이 되고 이 항원을 외부의 물질로 인지를 해서 그것에 대한 항체를 만든다. 그리고 그 다음번 임신에서 만약 Rh(+)을 가진 아이를 임신하게 되면 이 항체가 태반을 통해서 태아에게 건너가서 태아의 적혈구와 결합을 하여 적혈구의 파괴를 유도한다. 이렇게 적혈구가 많이 파괴가 되면 파괴된 적혈구에서 나오는 빌리루빈이라는 물질이 체내에 축적이 되고 이로 인해서 태아가 황달에 빠지고 또 이 물질이 뇌의 일부분에 침착이 되어서 뇌의 기능이상을 보인다. 많은 적혈구의 파괴에 의해서 빈혈이 생기는 것은 당연하다. 이것은 대개 Rh(－)인 사람이 한번이라도 Rh(+)인 사람의 혈액이 자신의 몸속에 들어와서 D에 대한 항체가 생겨야 하므로 대개 첫임신의 경우에는 생기지 않고 대신에 첫임신의 분만시에 Rh(+)의 혈액에 노출이 되어서 항체를 만들고 다음의 임신에서 Rh(+)의 혈액형을 가진 태아를 임신하는 경우에 생긴다. 이것의 예방을 위해서는 만약 Rh(－)의 산모가 Rh(+)의 아이를 임신한 경우에 분만에 특히 조심하여 상처를 통해서 태아의 적혈구가 산모의 혈액속으로 들어가는 것을 막는다. 그리고 분만 직후에 D에 대한 항체를 산모에게 주사를 하는 것으로도 예방을 하는데 이것은 태아의 적혈구에 있는 D항원과 이 주사한 항체를 결합시켜서 태아의 면역체계가 이 항원을 감지하지 못하게 하는 것이 그 메커니즘이다. |
