means a medical condition, Illness, injury or directly related medical condition, Illness or injury for which You required medical consultation, diagnosis, treatment or medication related to surgery, heart attack, heart failure, heart disease, angina, circulatory trouble, high blood pressure, stroke, cancer, kidney, liver or lung disease at any time within the 6 month period immediately prior to the Effective Date of coverage or date of departure on a trip.

A condition, whether physical or mental, regardless of the cause of the condition, for which medical advice, diagnosis, care, or treatment was recommended or received within the six-month period ending on the first day of coverage, or if there is an Eligibility Waiting Period, the first day of such Waiting Period. A Pre-Existing Condition does not include a Pregnancy when coverage is subject to the Health Insurance Portability and Accountability Act of 1996.

A Pre-Existing Condition is a condition for which medical advice; diagnosis, care or treatment was recommended or received within six (6) months of the person's enrollment date under this Plan. Genetic information is not a condition. Treatment includes receiving services and supplies, consultations, diagnostic tests or prescribed medications. In order to be considered treatment, the medical advice, diagnosis, care or treatment must have been recommended by or received from a physician.
