angle-closure g. glaucoma caused by closure of the anterior angle by contact between the iris and the inner surface of the trabecular meshwork; called also closed-angle g., narrow-angle g., and pupillary block g. Click here to view image■Glaucoma. Impairment of aqueous outflow is caused by closure of the anterior angle, with apposition of the iris and trabecular meshwork, in angle-closure glaucoma (A), and by other obstruction in open-angle glaucoma (B).

angle-closure g., acute the third phase of angle-closure glaucoma and a grave medical emergency. Initially the symptoms resemble intermittent angle-closure glaucoma. Then, as the intraocular pressure continues increasing, the cornea becomes swollen and steamy; the iris fixes in mid-dilation; there is excruciating ocular pain radiating to the other areas of the trigeminal distribution, and there may be nausea and vomiting. Finally the eye may become red and congested; visual acuity fails rapidly as the cornea keeps swelling, and severe visual field loss or even blindness may result if the intraocular pressure is not lowered.
