영문 cerebral contusion 한글 뇌좌상

  외부에서 기원하는 물리적 충격에 의한 뇌의 물리적 손상.

설명   
  맞거나 부딪쳐 생긴 상처. 피부 밑의 혈관이 터져 피부 밑조직에 출혈한 상태로 피부자체의 파열은 없다. 여러 가지 둔한 외력이 넓은 면에 가해졌을 때 생기는 상처로 충돌이나 추락 등에 의해 생긴다. 피부 밑의 피하조직-피하근막-근육 등의 조직에 널리 상처를 일으킨다. 환부에는 종창-동통-피하출혈 등을 볼 수 있고, 격통에 대해서는 필요하면 진통제를 쓰고 습포를 한다. 통증이 없어지면 온습포를 하고 부서진 조직의 흡수를 촉진시킨다. 사지 이외의 타박상에는 내장의 손상을 수반하며, 나중에 심각한 증상을 나타내는 경우도 있으므로, 신중한 검사를 필요로 한다.

  부딪혀 발생하는 일종의 멍. 즉 피부가 터지지 않은 상태에서 손상을 입은 상처. 특히 둔한 물제와 접촉하였을 때 생긴다. 이 경우 피부 밑의 혈관이 터져 피부밑조직에 출혈이 생긴다.

    생기는 부위에 따라 충격 부위 바로 밑에 나타나는 충격 좌상과 충격의 반대편의 뇌에 발생하는 반충격 좌상, 충격에 의해 뇌가 움직이면서 불규칙한 두개골의 내면에 부딪쳐서 발생하는 것들이 있다. 뇌회의 꼭대기에 주로 손상을 일으키며 대뇌 피질에 쐐기 모양의 결손을 초래하는데 피질의 최상층이 파괴되는 것이 경색과 다르다. 현미경 소견으로 급성 좌상은 출혈과 조직의 괴사를 보이며 진행되면 괴사된 조직은 탐식 세포에 의해 제거되며 공동을 만들고 그 주변에는 신경교증과 혈액에서 유래된 색소의 침착이 보인다.
