| 설명 | 선천심장병중의 하나. 태아가 뱃속에 있을 때는 혈액의 흐름이 출생후의 혈액순환과는 다르다. 태아는 호흡을 할 수 없으므로 산소가 있는 혈액을 단지 산모의 혈액에서만 구할 수 있다. 산모의 자궁동맥을 통해 태아의 배꼽정맥으로 넘어간 산화혈액은 간에서 정맥관(ductus venosus)을 거친뒤 심장에 이르게 된다. 심장에 이르른 혈액은 일부는 오른심방에서 곧바로 왼심방으로 가서 다시 좌심실을 거친뒤 대동맥으로 통하게 되지만, 일부는 우심방에서 우심실, 그리고 우심실에서 폐동맥으로 가는 정상출산아와 비슷한 혈액의 경로를 밟는다. 이때 폐는 자궁내에서는 호흡을 할 수 없으므로 폐의 작용이 없게 되고, 따라서 폐동맥으로 건너간 혈액은 폐를 거치지 않고 동맥관(ductus arteriosus)을 통해 곧바로 동맥에 이른다. 이런 태아의 순환은 출생후 호흡이 시작되면, 정상적인 출생 후의 순환으로 바뀌게 된다. 이 경우 태아순환에서만 존재하는 정맥관과 동맥관은 자연히 막히게 되고, 우심방에서 좌심방으로 흐르던 혈액의 순환은 없어진다. 그러나, 경우에 따라 태아순환에서만 유지되는 동맥관이 막히지 않는 병적인 상태가 발생하는데, 주로 미숙아, 혹은 선천성 바이러스 감염 등이 원인으로 추정된다. 이런 경우를 동맥관개존증이라 부르고, 심장은 불필요한 혈액의 흐름을 부담해야 하므로 일찍 심장기능상실(cardiac failure)에 빠지기 쉽다. 열려있는 동맥관의 크기가 작으면, 증상이 가벼워 늦게 치료해도 되지만, 큰 동맥관개존증은 빠른 수술적 치료를 하여야 한다. 일부에서는 저절로 막히기도 한다. |

| patent ductus arteriosus | <cardiology, embryology, paediatrics> A condition where the normal channel between the pulmonary artery and the aorta fails to close at birth. In normal foetal circulation the blood bypasses the pulmonary circuit since oxygen and nutrients are acquired through the placenta. After birth, this channel normally closes in response to ventilation of the lungs. Those who are affected may demonstrate poor growth, shortness of breath and rapid respiratory rate. Diagnosis is by echocardiogram and treatment involves the use of indomethacin to stimulate ductus arteriosus closure. | ductus arteriosus | Before birth, the blood headed from the heart (via the pulmonary artery) for the lungs is shunted away from the lungs and returned to the greatest of arteries (the aorta). The shunt is through a short vessel called the ductus arteriosus. When the shunt is open, it is said to be patent (pronounced pa'tent). The patent ductus arteriosus (PDA) usually closes at or shortly after birth and blood is permtted from that moment on to course freely to the lungs. If the ductus stays open (patent), flow reverses and blood from the aorta is shunted into the pulmonary artery and recirculated through the lungs.
