| 설명 | 고환종은 생식세포종양의 약 30%를 차지하여 단일 형태를 취하는 것 중 가장 흔하다. 고환종은 30대에 가장 많아서 생식세포종양 전체의 평균연령보다 다소 늦게 발생한다. ① 전형고환종(classic seminoma): 85%에서 고환이 커지고 정상 고환보다 10배까지 커질 수 있다. 현미경 소견에서 모양과 크기가 같은 균일한 종양세포와 림프구 침윤, 육아종 형성이 관찰된다. 출혈이나 괴사는 드물다. ② 역형고환종(anaplastic seminoma): 종양세포들의 크기와 모양이 불규칙하고 핵의 염색정도도 일정하지 않으며, 종양 거대세포의 출현과 세포분열이 전형고환종보다 흔하다. 가장 중요한 소견은 종양세포의 크기의 불규칙함과 현미경 400배 시야당 세 개이상의 세포분열이 관찰되는 점이다. ③ 정모세포고환종(spermatocytic seminoma): 전형고환종보다 훨씬 늦은 40대 이후 특히 65세 이후에 많다. 종양의 성장속도는 전형유형보다 느리고 전이도 드물다. 따라서 예후는 매우 좋다. 림프구 침윤이나 육아종 형성은 없다. |
