| 설명 | 사슬알균감염 후 생기는 심장판막병이다. 원인은 A군 -용혈사슬알에 의한 인두염후 일종의 면역반응으로 발병한다. 진단은 존의 기준에 의한다. (1) 주요기준은 관절염 심장염(심장비대, 심장잡음, 심장기능상실 등) 무도증: 무당이 춤을 추는 것 같은 행동의 발작증세. 연변홍반: 빨간 테두리를 가진 피부병변은 피하결절(subcutaneous nodule): 피부 밑에 생긴 결절, (2)참고 기준은 열, 관절통, EKG상 PR연장: 심전도 소견 급성기 반응물질(예: ESR, CRP)의 상승, 류마티스열 치료는 페니실린으로 치료하고 심장의 후유증 또한 페니실린으로 예방한다. |

| 설명 | 유전정보를 보관하는 물질인 DNA와 RNA 등을 이루는 핵산은 요산의 형태로 배설이 된다. 이것의 배설에는 여러 가지 효소가 작용한다. 이중에서 HGPRT (Hypoxanthine-Guanine Phosphoribosyl Transferase)라는 효소는 이미 배설이 되려고 요산으로 변환하는 과정의 물질을 다시 핵산으로 바꾸는 작용을 하는 효소이다. 통풍이란 이 HGPRT라는 효소 활성도가 감소되어 발생된다. 이 효소의 활성도가 감소되면 핵산의 배설단계에서 재사용이라는 단계가 없어지므로 과도한 요산의 생성이 있게 된다. 혈중에 과도한 요산이 존재하고 이 요산은 결정을 만들어서 관절 주위 및 연부 조직에 침착되어 격심한 발작성 관절통을 야기하는 급성 염증성 관절염을 일으키는 특징적인 병이다. 또 이 요산의 결정들이 콩팥이나 심장에 축적되어서 콩팥 및 심혈관계에 장애가 발생할 수도 있어 내과적 치료를 요하는 병이다. 유전 양식상 보통염색체 우성으로 유전하는 유전병이다. 보통염색체 우성 유전을 하지만 남자와 여자에서 표현률이 달라서 남자가 95%이상을 차지한다. |
