A nursing diagnosis is a clinical judgment about an individual, family or community response to actual and potential health problems/life processes. Data collected is analyzed to assist in formulation of the nursing diagnosis. The format of the nursing diagnosis is actual problem: Problem + Etiology + Defining characteristics/signs and symptoms (PES); and for potential problems: Problem + Risk factors (PR) (Kozier, et al., 1998).
