| 설명 | 근육의 동통. 어깨나 등과 같은 비교적 큰 근육에 많다. 근육 류마티스나 결합조직염 또는 어깨가 쑤시는 것과 비슷한 근육통증으로, 환부를 누르면 경결이 있고 몹시 아프다. 근육 자체에는 병적 변화가 없고 결합조직염의 경우는 그 근육 주위에 있는 근육막이나 힘줄-신경집 등이 결합 조직에 류마티스성 변화가 나타나므로 전형적인 비관절성 류마티스이기도 하다. 과격한 운동이나 몸에 배지 않은 일을 무리하게 했을 경우에 볼 수 있다. 증세로는 전신의 근육이 여기저기 쑤시고 아프다. |

| 설명 | 활동시에 근대약과 피로를 호소하다가 안정 후에는 회복되는 특징적인 증상을 호소하는 병으로, 근육과 신경접합부 사이 연결부분 이상으로 발생한다. 주로 젊은 여성에서 빈발하고, 눈주위 근육을 잘 침범한다. 대개 아침이나 휴식 후에는 호전되었다가 운동시 악화되는 피로현상을 보인다. 메커니즘은 아마 면역계의 이상으로 인해 발생하는 것으로 보이며, 치료 또한 면역을 억제하는 약을 다량 투여함으로써 호전되었다는 보고가 많다. 침범부위의 근육에 따라 여러 가지 종류가 있으며, 호흡근까지 침범하게 되면, 호흡기능상실로 사망한다. 신경-근육 접합부 중 접합뒤막의 아세틸콜린 수용체의 장애가 발생하는 자가면역병이다. 따라서 신경-근육 접합부에서 신경자극 전달이 차단되어 근육쇠약과 조기 피로가 온다. 환자의 85~90%에서 혈중에 아세틸콜린 수용체에 대한 항체가 있다. 전자현미경으로 관찰시 신경 근육 접합부 접합주름 감소 및 단순화를 보인다. 환자의 15~50%는 가슴샘종과 동반되고 나머지는 가슴샘과다형성과 잘 동반된다. 광학현미경상에서는 2형 근육섬유의 위축과 탈신경에 의한 작고 각진 근육섬유가 나타나는 것 이외에는 특별한 소견을 보이지 않는다. |

| 설명 | 미코박테륨속의 사람 결핵을 일으키는 균이다. 1882년 코흐(Koch)에 의해 발견되었다. 그램양성 산소성 막대균이며 0.3~0.6×2~4μm, 무아포, 항산성이다. 항산, 항알칼리, 항알코올, 소독제에 저항한다. 저항력과 번식력이 강하여 전염성이 높으나 건조, 열, 햇빛에는 약하다. 세포벽에는 다량의 지방질이 포함되어 있다. 니아신검사는 사람형 결핵균만 양성이며 이것은 감별에 도움이 된다. 감염은 비말감염이고 폐결핵이 많지만 혈행 속에 들어가면 모든 장기에 결핵을 일으킨다. 특유한 결핵결절을 형성한다. 결핵의 진단에는 트베르쿨린 반응이 사용되며 예방에는 비씨지(BCG) 접종이 실시된다. 면역은 세포성 면역이며 큰포식세포의 활성화로 인해 균의 증식이 억제된다. |
