| 설명 | 주로 소아에서 일어나고, 그 결과로 지속성 면역을 주는 전염성의 paramyxovirus 병. 흡기에 의해서 감염되는데, 가장 농후한 바이러스감염은 침샘내에 생기며, 특히 턱밑샘이나 혀밑샘보다도 귀밑샘이 더욱 심하다. 잠복기는 18~22일이다. 감염은 약 75%의 증례에서 증상을 나타낸다. 이들 중 귀밑샘염은 70%에서, 수막염은 10~15%(이들 중 반수에서 무증상성 수액세포 증가증이 동반된다)에서 일어난다. 부고환고환염이 사춘기 후의 남성에서 생기지만 그 후에 불임증을 속발하는 일은 드물다. 그 외의 증상은 보다 드물게 나타나는데 이자염, 관절염, 심근염, 난소염, 갑상샘염 및 젖샘염 등이 있다. 발열과 환부의 유통성 염증은 최초의 2일간에 가장 현저하고 다음 4~5일이 지나면 서서히 가라앉는다. 동시에 한 부위 이상을 침습하는 수도 있으며 가끔 속발적으로 침습되고 질환의 전 경과는 2~3주에 이른다. 부수되는 영속성의 신경학적 손상을 동반하는 수막뇌염은 드물다. |

| 설명 | 신체를 싸고 있는 바깥층으로서 표피, 진피, 피하조직으로 구성되며, 외부 환경으로부터 신체를 보호하고 체온을 조절하는 기능을 가짐. 1.표피: 피부의 가장 바깥층으로서 외배엽에서 기원하며 각질이라는 구조 단백질을 생산하는 기능을 가짐. 4가지의 세포가 있는데, 각질세포, 멜라닌세포, Langerhans 세포, 부정형 세포, Merkel세포가 있다. 표피에는 이 4가지 세포외에 다음과 같은 표피 부속기가 있다. 에크린 땀샘(eccrine sweat gland): 점막 피부 경계 부위 등 특수한 부위를 제외한 모든 피부에서 발견되며 땀을 분비하는 샘으로서 신체 열 조절 체계의 중요한 요소. 아포크린샘(apocrine gland): 겨드랑이, 젖꽃판, 항문 생식기 부위, 외이도, 눈꺼풀 등에서만 발견되며 분비물의 기능은 잘 알려져 있지 않으나 동물에서는 방어 및 성적 기능이 있음. 털주머니(hair follicle): 손바닥, 발바닥 등 특수 부위를 제외한 신체 전 부위에 분포하며 인간에서 중요한 생리적 기능은 없으나 성적 매력을 지니는 신체 장식의 기능을 가짐. 기름샘(sebaceous gland): 손바닥과 발바닥을 제외한 전 피부에 분포하며 분비물을 기름(sebum)라 부름. 남성 호르몬의 자극에 의해 성장하며 생리적 기능은 잘 알려져 있지 않음. 손발톱(nail) 2. 진피: 표피 아래에 존재하는 중배엽 기원의 조직으로서 혈관, 림프관, 신경, 근육, 표피 부속기 등을 포함하고 있음. 표피 바로 밑의 부위를 유두 진피라 하고 아래층을 망상 진피라 한다. 조직 섬유에는 아교질 섬유, 그물 섬유, 탄력 섬유의 세 종류가 있다. 3. 피하 조직: 진피 아래에 존재하며 중배엽 기원으로서 보온 작용, 충격 흡수 작용, 영양 저장 작용의 기능이 있다. |

| mumps | An infectious acute viral disease affecting the parotid glands. The gonads, meninges and pancreas can also be affected. The causative agent is a paramyxovirus. Humans are the only natural host for this disease. Common symptoms include weakness, fever, sore throat, malaise and puffiness to the cheeks (due to parotid gland swelling). Patients are contagious 1 day prior to the onset of swelling until the swelling is gone. Recovery is generally in 2 weeks. (27 Sep 1997) |
| mumps immunization | The standard MMR vaccine is given to prevent measles, mumps and rubella (german measles). The mmr vaccine is now given in two dosages. The first should be given at12-15 months of age. The second vaccination should be given at 4-6 years (or, alternatively, 11-12 years) of age. most colleges require proof of a second measles or mmr vaccination prior to entrance. Most children should receive mmr vaccinations. Exceptions may include children born with an inability to fight off infection, some children with cancer, on treatment with radiation or drugs for cancer, on long term steroids (cortisone). People with severe allergic reactions to eggs or the drug neomycin should probably avoid the mmr vaccine. Pregnant women should wait until after delivery before being immunised with mmr. People with HIV or aids should normally receive mmr vaccine. Measles, mumps, and rubella vaccines may be administered as individual shots, if necessary, or as a measles-rubella combination. | mumps in pregnancy | It has been stated, we believe erroneously, that mumps is dangerous when contracted during pregnancy. For example, vetter (infect med 14:730-733, 1997), citing a single 1980 article, writes: mumps infection during the first trimester of pregnancy can increase the rate of spontaneous abortion. Congenital anomalies associated with mumps infection during pregnancy include endocardial fibroelastosis; imperforate anus; spina bifida; and auditory, optic, and urogenital deformities. However, shepard in his authorative catalog of teratogenic agents (j hopkins u press, 8th edition, 1995) does not consider that mumps merits inclusion as a proven or even possible teratogenic agent, that is as an agent capable of causing a spontaneous abortion (a miscarriage) or causing congenital malformations (the baby is born with birth defects). Furthermore, mumps does not even warrant mention in smith's recognizable patterns of human malformation (by kl jones, wb saunders co., 5th edition, 1997), a key standard text on dysmorphology (the study of malformations). Based also on our review of the facts available at this time (oct, 1997), we conclude that there is insufficient evidence to label mumps as a particular hazard in pregnancy. (12 Dec 1998) |
