| 설명 | 전반적인 근육의 약화와 동반하여 서 있을 수 없고 의식을 소실하는 상태를 말함. 1) 메커니즘: 실신이 일어나는 메커니즘은 의식을 관장하는 대뇌부위의 산소결핍에 의해서 일어난다. 실신시 뇌혈류의 감소와 대뇌산소이용의 감소를 보인다. 2) 원인 (1)미주신경반사성 실신(vasovagal syncope): 이것은 일반인에게서도 일어날 수 있는 실신으로서 감정적 격앙에 의해서 갑자기 정신을 잃게 된다. 부교감 신경인 미주신경의 흥분에 의해 혈압이 하강함으로써 유발됨. (2)체위성 저혈압(postural hypotension): 혈관운동성 반사의 결합이 있는 사람은 앉아 있다가 일어날 때 갑자기 혈압이 떨어지게 되는데 이런 경우 실신이 동반될 수 있다. (3)심장성 실신(cardiac syncope): 부정맥에 의하여 심박출량의 갑작스런 감소에 의해서 유발되는 실신으로 스톡스-아담스 증후군(Stokes-Adams syndrome)이라고도 불림. (4)경정맥동증후군(carotid sinus syndrome): 목정맥동(carotid sinus)은 혈압에 대한 수용체가 존재하여 혈압이 너무 높을 때 뇌에 신호를 보내어 심박출량을 줄이고 혈압을 낮추는 기능이 있는데 이러한 목정맥동이 혈압이 낮은데도 작용하는 병적상태를 말함. 따라서 낮은 혈압에도 심박출량을 줄여 실신을 유발함. |

| anaesthetics, local | Drugs that block nerve conduction when applied locally to nerve tissue in appropriate concentrations. They act on any part of the nervous system and on every type of nerve fibre. In contact with a nerve trunk, these anaesthetics can cause both sensory and motor paralysis in the innervated area. Their action is completely reversible. Nearly all local anaesthetics act by reducing the tendency of voltage-dependent sodium channels to activate. They are commonly used not only in the peripheral nervous system, but also for spinal anaesthesia.
