isoprenaline Isoprenaline is absolutely contraindicated in all cardiac arrests irrespective of cause (other than torsades de pointes). Its unopposed beta adrenergic activity not only increases myocardial oxygen demand but also reduces peripheral vascular resistance, lowers coronary perfusion pressure and reduces coronary arterial blood flow. Isoprenaline may be used to accelerate a bradyarrhythmia, but only in the presence of a cardiac output.
