| 설명 | 산소운반과 활용 과정에 장애를 발생하여, 동맥혈 산소함량(PaO2)이 감소하며, 조직 속의 산소가 정상치 이하로 감소한 상태. 대별하면 ① 저산소성저산소증, ② 빈혈저산소증, ③ 울혈저산소증, ④ 조직독성저산소증, ⑤ 수용성 저산소증을 분류된다. 초기증상은 의식혼란, 두통, 어지럼증, 구역, 가쁜 호흡, 빠른 맥 등으로, 증상이 진행하면 청색증(혈증 환원 Hb 가 5g/100mL 이상), 의식소실, 경련, 느린맥, 부정맥, 호흡억제, 심장정지에 이른다. 산소요법, 기계적 인공호흡을 실시한다. 그 종류를 원인별로 분류하면 다음과 같다. ① 저산소성저산소증(hypoxic hypoxia): 동맥혈의 산소분압이 감소되어 초래된 저산소증으로 흡입 공기 중의 산소 농도가 감소한 경우, 호흡 중추억제-기도폐쇄-호흡근마비 등 폐포환기가 감소한 경우, 무기폐-폐렴-폐부종 등 폐포에서의 가스 교환이 부적절하여 일어나는 경우, 고열-신진대사항진 등 조직의 산소 소모가 증가하여 일어나는 경우, 이산화질호(N2O)를 이용한 전신마취 후에 드물게 나타나는 확산성 저산소증 등에 의해 발생할 수 있다. ② 빈혈저산소증(anemic hypoxia) : 혈색소가 정상 이하로 감소한 경우, 일산화 탄소중독처럼 혈색소가 산소를 운반할 수 없는 경우에 나타나는 저산소증으로서 산소분압은 정상 범위 내에 있으나 동맥혈의 산소 함량이 크게 감소되어 있는 경우가 대부분이다. ③ 울혈저산소증(stagnant hypoxia) : 동맥혈의 산소분압은 정상이나 조직의 혈액순환이 장애를 받아 나타나는 저산소증으로 심박출량이 감소하여 오는 경우도 있고, 색전증-외상-혈관수축-정맥폐쇄 등에 의해 발생할 수도 있다. ④ 조직독성저산소증(histotoxic hypoxia) : 사립체의 산소 이용률이 낮아진 경우에 나타난다. 혈관 확장제를 과량 사용하게 되면 세포 내 효소의 독성에 의해 저산소증이 올 수 있다. |
