This is a legal document in which an individual designates another person to make health care decisions if he or she is rendered incapable of making their wishes known. The health care proxy has, in essence, the same rights to request or refuse treatment that the individual would have if capable of making and communicating decisions.

A written instrument which names another person, called the "agent," to make health care decisions for him or her when a doctor certifies in writing that the person can no longer make health care decisions for himself or herself. This document becomes operative only when the person becomes incapacitated in the attending physician's opinion and unable to understand the nature and consequences of health care decisions. A person may revoke a Health Care Proxy verbally or in writing.
