| 설명 | 장관에서 영양소가 혈액으로 잘 흡수되지 않는 현상. 작은창자에서 영양소가 일부 또는 전부 흡수되지 않아 생기는 병을 말한다. 이 병은 대부분 매우 천천히 발병되기 시작하여 진단하기가 매우 어려우며, 병을 의식하기 전의 수개월에서 수년 동안 진행되는 경우도 있다. 장기간 치료해야 하며, 치료방법 복잡하고 자주 바뀔 수 있다. 남-녀 모두 비슷한 비율로 모든 연령층에서 발생할 수 있다. 원인은 첫째, 1차적 선천적 이상으로서 젖당-설당 등 이당류 분해 효소의 결핍, 이자-작은창자 등의 소화효소 결핍, 포도당과 비타민 B12 등 작은창자 점막의 전송장애 등을 들 수 있다. 둘째, 2차적 원인으로서 창자병 등으로 계속 흡수불량이 일어나는 경우이다. 이 밖에 창자관 내의 효소가 부족하거나 정상 상태의 창자병 세균이 변하는 경우, 이자-간-쓸개 등의 병으로 소화가 잘 되지 않는 경우, 기생충-복통 등 창자관 벽의 병이 있는 경우, 창자관절제 수술로 흡수 면적이 줄었을 경우 등을 들 수 있다. 위험 인자로는 알코올 과다섭취, 창자관수술, 가족 중 흡수불량이나 낭포성섬유증 환자가 있는 경우, 광유 또는 다른 완하제를 사용한 경우 등이 있다. 증세는 식욕부진-불쾌감-체중감소-설사-복부팽만 등이 나타나고, 단백질-지방-비타민 등의 흡수불량으로 각각의 결핍증세가 나타난다. 단백질 결핍에 의해서 유아의 발육부진, 빈혈, 저알부민혈증, 부종 등이 생기며, 비타민 D의 흡수장애로 인하여 뼈의 발육에 문제가 생긴다. 비타민 K와 B12의 흡수장애로 출혈과 거대적모구성빈혈이 생기고, 작은창자 점막의 포도당과 갈락토오스의 전송 장애로 인해 설사-저혈당-영양장애 등이 나타난다. 또한 무기력과 경미한 빈혈 등이 나타나고, 복부에 가스가 차거나 막연히 불편하며, 대변의 냄새가 나쁘고 양이 많다. 합병증으로 소화불량이 장기간 지속되거나 유아의 경우 사망할 수 있으며, 영양소-비타민-무기질 결핍 등으로 인하여 다른 질환이 생길 수도 있다. |
