| 설명 | 분만시에 태아의 머리에 가해지는 압박을 최대한으로 줄이고 분만 시간을 단축시키고, 외음부(항문과 음문사이의 부분)의 상처를 예방하기 위해서 분만시에 외음부 및 질벽의 일부를 절개하는 것을 말한다. 태아의 머리가 음문을 통과할 때는 회음부가 갑자기 늘어나게 되므로 회음부가 찢어지기 쉽다. 따라서 회음보호를 행하는데, 회음의 신전성이 불량하고 아이의 머리가 지나치게 크거나 태아의 분만을 신속히 처리해야 할 때는 회음부의 손상을 방지할 수 없다. 이와 같은 경우는, 진통발작으로 회음이 강하게 긴장해 극도로 얇아졌을 때 재빨리 회음의 중앙을 항문을 향해 절개하거나, 또는 회음중심부에서 1.5~2cm 옆쪽 부분을 비스듬히 절개한다. 전자를 정중외음절개술(median episiotomy), 후자를 중외측외음절개술(mediolateral episiotomy)이라고 한다. 외음절개술은 모체의 보호뿐만 아니라 태아 머리에의 압박을 경감시키는 장점이 있다. |
