| 설명 | Na+과 수분의 배설을 촉진시킴으로써 소변량을 증가시키는 약물을 이뇨제라 한다. 즉 소변의 양을 증가시키는 약물을 이뇨제라고 한다. 소변을 생성하는 과정은 크게 2가지로 대별된다. 소변은 콩팥에 약 100만개 존재하는 콩팥단위(nephron)이라는 기능의 단위에서 만들어지는데 이 콩팥단위은 토리와 뇨세관으로 이루어진다. 토리란 콩팥으로 들어오는 혈액을 걸러내는 곳으로 우선 이곳에서 일단 걸러진 것을 원뇨라고 한다. 뇨세관은 이 원뇨가 지나가는 곳으로 이 원뇨에서 신체에 필요한 성분은 다시 체내로 흡수하고 토리에서 걸러지지 않은 불순물을 분비하여 오줌을 만드는 역할을 하는 곳이다. 이뇨제의 작용기전은 역시 이 2가지 과정에 관계하는데 토리에서 걸러지는 원뇨의 양을 증가시키는 기전과 세뇨관에서 재흡수를 하는 과정에 관계하여 수분과 Na+의 재흡수를 억제하는 기전으로 크게 나눌 수가 있다. 전자의 메커니즘으로 작용하는 이뇨제로는 심장에서 나오는 혈액의 양을 증가시켜서 콩팥의 토리를 지나는 혈액양을 증가시켜 많은 혈액을 걸러서 많은 양의 원뇨를 만드는 강심이뇨제와 삼투압을 증가시키는 물질을 투여하여 토리에서 걸러지는 양을 늘이는 삼투성 이뇨제 등이 있다. 현재 일반적으로 사용되는 대부분의 이뇨제들은 후자에 속하는, 즉 콩팥뇨세관에 작용하는 것들이다. |

| 설명 | 적혈구세포의 삼투압 특성 변화에 대한 민감성. 적혈구가 저농도의 나트륨 용액에 노출하면 세포막의 능력을 초과할 때까지 다량의 물을 흡수하여 부풀어올라 파열하고 고농도의 나트륨 용액에 노출하면 적혈구의 세포내액이 빠져나가 크기가 줄어들며 파괴된다. 예외적 취약성이나 저항성은 특정 병의 진단적 기준이 될수 있다. 정상세포는 0.39~0.45% 소금물 용액에서 용혈이 시작되며, 37℃(98.6℉)에서 0.30~0.33% 소금 용액에 적혈구를 노출시키면 용혈은 24시간 이내에 완료된다. |

| diuretics, osmotic | Diuretics that are filtered at the glomerulus and reabsorbed in the renal tubule only to a limited extent. They thus increase the amount of osmotically active solute in the urine and a corresponding increase in urine volume. These compounds also increase the osmolality of plasma, thereby increasing the diffusion of water from the intraocular and cerebrospinal fluids; they can be used for reducing the pressure and volume of these fluids.
