| 설명 | Na+과 수분의 배설을 촉진시킴으로써 소변량을 증가시키는 약물을 이뇨제라 한다. 즉 소변의 양을 증가시키는 약물을 이뇨제라고 한다. 소변을 생성하는 과정은 크게 2가지로 대별된다. 소변은 콩팥에 약 100만개 존재하는 콩팥단위(nephron)이라는 기능의 단위에서 만들어지는데 이 콩팥단위은 토리와 뇨세관으로 이루어진다. 토리란 콩팥으로 들어오는 혈액을 걸러내는 곳으로 우선 이곳에서 일단 걸러진 것을 원뇨라고 한다. 뇨세관은 이 원뇨가 지나가는 곳으로 이 원뇨에서 신체에 필요한 성분은 다시 체내로 흡수하고 토리에서 걸러지지 않은 불순물을 분비하여 오줌을 만드는 역할을 하는 곳이다. 이뇨제의 작용기전은 역시 이 2가지 과정에 관계하는데 토리에서 걸러지는 원뇨의 양을 증가시키는 기전과 세뇨관에서 재흡수를 하는 과정에 관계하여 수분과 Na+의 재흡수를 억제하는 기전으로 크게 나눌 수가 있다. 전자의 메커니즘으로 작용하는 이뇨제로는 심장에서 나오는 혈액의 양을 증가시켜서 콩팥의 토리를 지나는 혈액양을 증가시켜 많은 혈액을 걸러서 많은 양의 원뇨를 만드는 강심이뇨제와 삼투압을 증가시키는 물질을 투여하여 토리에서 걸러지는 양을 늘이는 삼투성 이뇨제 등이 있다. 현재 일반적으로 사용되는 대부분의 이뇨제들은 후자에 속하는, 즉 콩팥뇨세관에 작용하는 것들이다. |
